Memorandum of Agreement
(MOA)

Know all men by these presents:

This Memorandum of agreement (MOA) is made and entered into this ______day of
________by and between:

The Ildefonso- Gamalinda Memorial Homes, with office address at Rizal Avenue, Tuburan
District, Pagadian City, herein referred to as the funeral homes,

And

The municipality of Margosatubig, Zamboanga del Sur herein represented by Hon. George M.
Minor in her capacity as Mayor and hereafter referred to as the Municipality;

WITNESSETH THAT

Article I. ROLES AND RESPONSIBILITIES OF THE PARTIES

Section 1. The roles and responsibilities of the funeral homes;

Pursuant to this agreement, it shall be the role and responsibility of the funeral homes to;

1. In times of calamities/insurgencies, may it be natural or man-made, supply coffins/ caskets

and to provide other burial and funeral services to bereaved families of victims/casualties
cause by the disaster as determined by MDRRMC;
2. That the funeral homes shall exhaust all remedies to supply the needed number of coffins for
the victims/casualties for proper entombment;
3. Agree that payment will be made after the crisis or upon release of the MDRRM Fund by the
Government;

Section II. The Roles and Responsibilities of the Municipal Government;

Pursuant to this agreement it shall be the role and responsibility of the municipal government
to undertake the following;

1. In times of calamities, the MDRRM Chairman shall convene the MDRRM Council to
determine the intensity of the damage. To identify the number of the affected
families/individuals or the number of the casualties/victim;
2. To evaluate how much will be release out of the MDRRM Fund to respond to the funeral
needs of the affected families;
3. The municipal Government upon evaluation shall communicate with the funeral home about
the cost of the funeral services to come up with the approximate amount to be released;
4. Release funds for rehabilitation, livelihood Programs to bereaved family and sufficient
amount to support the victims funeral expenses;
5. Shall exhaust all efforts to Fast tract the release of fund for payment of funeral services;

Article II. Disengagement, termination for cause, and sanctions

 1. The Municipal Government may request termination of this MOA and upon written notice to
the other parties specifying the reason of disengagement.
2. Non-compliance of this agreement, shall mean termination of this MOA

Article III. Affectivity and Amendments.

This Memorandum of agreement shall take effect on ___________________________,

unless sooner be terminated by both parties, in writing.

Any amendment to this agreement shall be made in writing and must be concurred by both
parties.
