SUBJECT: REQUEST FOR OVERLOADING

Dear Sir/Madam:

I am John Roy F. Tionko, a 4th year Civil Engineering student, humbly requesting for your consideration in allowing
me to overload units for this semester. (or overload the allowable number of units to enroll for the following course/s):

1. CE017, Quantity Surveying – 2 units 5. CE021, Principles of Earthquake Engineering – 2 units

2. GEC008, Ethics – 3 units 6. CE410, Hydrology – 2 units

3. GEC002, Readings in Philippine History – 3 units 7. CE414, Integration Course 2 for CE – 2 units

4. GEE003, General Education Elective 3 – 3 units

The primary reason for my request is I can’t enroll these courses because I failed the course CE414 Integration
Course for CE 2 so I am not yet a graduating student. I want to enroll these courses because I can only afford 6 units
from the previous courses from the last semester. I don’t want to waste the allowed units in this semester so I am
asking you Sir/Madam a favor to please add/open these courses and I’ll promise that I will pass all these courses.

Attached in this request letter are my current pre-enrollment form and my current curriculum checklist generated from
my ARIS Account.

I look forward to your positive response regarding my request. Thank you and God bless.

Sincerely,

JOHN ROY F. TIONKO
