JOINT AFFIDAVIT OF TWO DISINTERESTED

PERSONS

We, RITA L. DIOLA and NORMA B. NAVARRO, both of legal age,

Filipino citizens, both married, and residents of Zone 4, San Agustin, Dulag,
Leyte and Brgy. Magsaysay, Dulag, Leyte after being duly sworn in accordance
with law, hereby depose and state that:

1. We personally know ASHLY VILLETE SELVIO, a resident of Brgy. San

Agustin, Dulag, Leyte who is the daughter of our longtime friend
ANGELIE VILLETA CASIPONG;

2. We know for a fact that the person by the name of “ASHLY VILLETE
SELVIO” as appearing in some documents, specifically in her Certificate
of Live Birth and the name “ASHLY CASIPONG SELVIO”, refer to one
and the same person;

3. We are not in any way related to any of the foregoing person within the
fourth civil degree either by consanguinity or affinity, nor do we have any
pecuniary interest in this matter whatsoever;

4. To remove confusion as to the two names, we hereby declare that the

name ASHLY VILLETE SELVIO and ASHLY CASIPONG SELVIO refer
to one and the same person whose registered name is ASHLY
CASIPONG SELVIO;

5. We are executing this affidavit to attest to the truthfulness of the foregoing

statements and to establish the real identity of ASHLY CASIPONG
SELVIO, and for all legal intents and purposes it may serve.
