I BOYA SUDHAKAR S/O B. GIDDAIAH aged about 32 years, resident of H.NO

1-134, PUSULUR, KALLUR, KURNOOL. Do hereby solemnly affirm and state as
follows:

1. That my FATHER BOYA GIDDAIAH S/O BOYA BADE SAV Died, on

18/02/2016 at H.NO. 1-134, PUSULUR, KALLUR, KURNOOL and he/she is
now aged 56 years.
2. That the matter was not reported to the Municipal Corporation
Authorities/Gramapanchayath hence there is no official record in the office of
the Municipal Corporation Kurnool. Vide their Non availability Certificate
ID.NO. , Date .
3. That I Want to take Date of Death Certificate of my FATHER BOYA GIDDAIAH
S/O BOYA BADE SAV from the Tahsildar, KALLUR to enter the Date of Death
in Municipal Records, Kurnool.
4. Hence I request the Tahasilder, KALLUR the Date of Death Certificate may
please be given to me of my FATHER BOYA GIDDAIAH.
