Lampitak National High School is planning to conduct a Five-Day Midyear


Performance Review and Evaluation (MPRE) Week on March 15-19, 2021 at Lampitak
National High School. This activity aims to give teachers additional knowledge for their
professional growth through various topics to be discussed. This activity will also
evaluate the progress attained in the implementation of the educational program of the
school, and review the performance of teachers and the school staff with a view to
addressing concerns through cooperative effort.

Activity Design for a Five-Day Midyear Performance Review and Evaluation


(MPRE) Week

I. Title of Activity: Five-Day Midyear Performance Review and Evaluation (MPRE)


Week

II. Rationale:

The Midyear Performance Review and Evaluation (MPRE) is conducted every


semestral break in accordance to DepEd Order 36, s. 1997. This order is pursuant to
theguidelines of DECS Order No. 1 and 2, s. 1993. Outputs of such activities may be
instructional materials, teaching techniques, etc. based on their needs as revealed in
the performance appraisal conducted. It also stated that it shall be considered as a
regular part of the curricular activities. The MPRE week is a way of assessing and
intervening the lacking skills and needs of teachers for the improvement of the
institution.
With this intention, we deemed it right to hold a Five-Day Midyear Performance
Review and Evaluation (MPRE) Week on March 15-19, 2021 at Lampitak National High
School. This activity aims to evaluate the progress attained in the implementation of the
educational program of the school, and review the performance of teachers and the
school staff with a view to addressing concerns through cooperative effort.

III. Objectives:

The activity aims to;

1. Enhance teachers knowledge and back them up with appropriate in-service


learning;
2. Identify the needs of teachers towards their teaching;
3. Build and strengthen camaraderie among the teachers, staff and school head;
4. Maximize utilization of school resources which contribute to the development
and improvement of the institution, the teachers, students, and stakeholders.

IV. Persons’ Involve:


This activity will involve School Head and teachers.

V. Source of Fund:
Expenses are chargeable against the MOOE Funds.

VI. Budgetary Requirements

VI. Expected Output:

Activity Design for a One-Day United Nations Month Culmination Program

I. Title of Activity: United Nations Month Culmination Program

II. Rationale:

One of the significant activities in school is the celebration of the United Nations
Month during the month of October. The point in time we remember the founding of the
United Nations wherein the Philippines is one of the original member. The United
Nations as an organization of countries that agree to cooperate with one another and
brings together equality before the international community.

The school as an institution of learning and in order to promote the aims and
objectives of the United Nations initiates activities for students to participate as well as
develop awareness on the vital role of the organization in maintaining world peace.
Towards this end, Lampitak National High School finds it a necessity to commemorate
the United Nations Day on October 31, 2014 at the school covered area.

III. Objectives:

The activity aims to;

5. Increase the knowledge of students about the role of the United Nations;
6. Improve students critical thinking about the functions of the United Nations on
current international issues;
7. Develop students with a sense of responsibility as citizens of the country vis-
a- vis member country in the United Nations.

IV. Persons’ Involve:

This activity will involve students, teachers and invited guests.

V. Source of Fund:

Expenses are chargeable against the SSG Local Funds.

VI. Expected Output:

At the end of the activity, the participants are expected to;

a. understand the role and functions of the United Nations;


b. acquainted with the member countries in the United Nations;
c. developed critical thinking about the international community and international
relations; and
d. act responsibly in accordance with the laws of the United Nations.

I. Title of Activity: School-Based World Teachers Day Culmination Program

II. Rationale:

One of the significant activities in school is the celebration of the World Teachers
Month which culminates every October 5. The point in time we remember our mentors
of their greatest contribution in transforming our lives to become holistic person. Indeed,
teachers are the silent heroes of the modern times.
Teaching as a profession requires exemplary personality and competence to
model the students. The teacher must at all times prepared of his lessons to deliver the
best services to the students. Thus, to be a teacher is a profession that deserves
special recognition for them lies the future of the students and the country.

The Supreme Student Government of Lampitak National High School is planning


to conduct a school-based culmination program of the World Teachers Month on
October 5, 2016 at Lampitak National High School Campus with the theme “MY
TEACHER, MY HERO” This program aims at encouraging the students to appreciate
and recognize the teachers heroic contribution in their educational journey.

III. Objectives:

The activity aims to;

1. Increase the knowledge of students about the role of the teachers in nation
building;
2. Develop students’ appreciation to teaching as a profession.
3. Recognize the teachers’ contribution in transforming the lives of every student to
become successful in the future.
4. Send messages of thanks and appreciations to the teachers.
5. Mobilize student leaders for classroom mentoring to provide space for teachers.

This activity will involve students, teachers, retired teachers and invited guests.

II. Rationale:

The stakeholder is the enormous partner of the school in realizing the vision of
the Department Education. Their contribution is attributed to the development of the
students and the way the school operates. Broadened & strengthened stakeholder
support in the improvement of basic education outcomes help teachers and
administrators to effectively deliver quality education to the clientele.
The Faculty and Staff Association of Lampitak National High School in
partnership with the Parents Teachers Association and Supreme Student Government
deemed it necessary to conduct activities that will strengthened the commitment and
active participation of stakeholders in the delivery of services for the general welfare of
the school.
With these intention, we deemed it right to hold a One-Day Family Day and
Recognition of Outstanding Stakeholders on December 20, 2016 at Lampitak National
High School Covered Area. This activity aims to recognize the indispensable
partnership of the stakeholders in sustaining the best practices and services to the
clientele and helping them achieve the apex of their educational journey.
