Affidavit of loss

I, Ereboy Arsenal, of legal age, single, resident of Sitio Bucroz Dacu Brgy. Ayungon La Carlota City, Negros
Occidental, after having been duly sworn in accordance with law, do hereby depose and say:

1.) That I am a Registered Police Officer assigned in La Carlota City Police Station;

2.) That as a Police Officer I was issued a Police License issued by the PNP with an license no. 09876aa;

3.) That in May 1, 2023 when I was patroling around the City Plaza of La Carlota I saw bunch of people
shouting chaosly that there's a theft, I was then chase the suspect with my patrol bike but he goes to
skwater area of Sitio San Agustin, Brgy Dos, where my bike could not pass through;

4.) That said situation I haven't any choice but to chase him by running, I left my Patrol Bike outside the
passage of the skwater community in Sitio San Agustin, Brgy. Dos;

5) That I finally arrested the suspect, and went back to
