AFFIDAVIT OF EXEMPTION

I, ___________, of legal age, married, and a resident of _____________, Philippines,

after having been duly sworn to in accordance with law hereby depose and say:

1. That my last steady employment was with the __________ where I worked from
_____ to _________ as a _______;

2. That after my resignation from said office I continued with my college studies under
the support of my elder sister as a part-time student;

3. That other than the minimal support from my sister I engage in selling several kinds
of products ranging from herbal food supplements to real estate from which I could
occasionally get compensation to pay for other expenses;

4. That I have recently graduated from Gregorio Araneta University Foundation with a
degree in Bachelor of Science in Agricultural Engineering just this April 13, 2002 and
is currently looking for a job;

5. That my earnings for the past years since 1999 cannot be considered substantial for
me to file an income tax return from the Bureau of Internal Revenue and thus
exempted from filing as per National Internal Revenue Code (R.A. 8424) Chapter IX,
Sec. 51, Par. 2.b;

6. That a Certificate of Exemption was issued by the Barangay of Karuhatan, City of

Valenzuela, where I am a resident since birth, concurring with aforesaid claim for
exemption;

7. That I am executing this affidavit to attest to the truth of the foregoing circumstances
and for the purpose of complying with the scholarship requirement of my daughter
__________ at the Philippine Science High School.
