MINUTES OF MEETING
PRESENT:

GRADE 10 PARENTS

AGENDA:
1. COMPLETION OF ATTACHMENTS TO FORM 137.
2. IMPLEMENTATION OF APPROVED VERIFICATION FORM TO COMPLETING STUDENTS.
3. PICTURE PACKAGE FOR THE COMPLETING STUDENTS.
4. CITATION OF EXPENSES FOR THE COMPLETION DAY.
PROCEEDINGS:
1. Meeting was called to order at 2:00 pm by the Grade 10 year level Chairman, Mr. Ed Quinto.
2. The first part of the meeting was formally started with singing of the Philippine National
Anthem, followed by a short prayer all through audio presentation. Participants were
welcomed by the year level chairman, Mr. Ed Quinto. The grade 10 advisers were formally
introduced by Mrs. Alma E. Paloma.
3. Mrs. Evelyn E. FabrIgas , the principal of Agdangan National High School delivered
inspirational message to all the participants.
4. Mrs. Jovelyn Bayos, Mrs Wena Barrameda and Mr. Ed Quinto discussed the following
important matters:
a. Reminders on the completion of attachments to form 137.
b. Citing of the importance of the approved verification form.
c. Inclusions on picture packages.
d. Over-all expenses for the upcoming completion day.
ACTION TAKEN
Teacher advisers finalized on the following:
a. Attachments will be submitted on or before the 2 nd week of June 2023.
b. Verifications form should be completed on or before June 30,2023 or a week before
completion day.
c. Parents are free to choose on picture package they preferred to.
d. Parents agreed to pay the contribution for the completion day.

Meeting was adjourned at 3:47 P.M.
