    To whom it may concern:

    I, JESSIE M. DELA CRUZ, hereby authorize CHRISTIAN C. MUNDO to claim my CNA for I am
    unavailable due to the distance of my current workplace to your esteemed institution. As much
    as I would like to claim it myself, I also could not personally come to your office due to my work
    schedule during weekdays.

    I have ended my contract for the First Semester of the Academic Year 2022-2023 last January
    2023. After that, I did not renew my contract of service to your institutions since I have already
    received my Plantilla item in the Department of Education.

    I am hoping for your consideration in such situation.
