DEED OF UNDERTAKING

KNOW ALL MEN BY THESE PRESENTS:

This Deed of Undertaking is executed this ______________ day of _____________________ at

_________________________ by :

VERSOZA, RICHARD SINDREJAS, of legal age, Filipino citizen, and a resident at BLK
52 LOT 24 LAPU-LAPU ST. UPPER BICUTAN TAGUIG METRO MANILA, hereinafter referred to
as the "Borrower";

in favor of

SECURITY BANK CORPORATION, a corporation duly organized and existing under and
by virtue of the laws of the Republic of the Philippines, with business address at Security Bank
Centre, 6776 Ayala Avenue, Makati City, hereinafter referred to as “Bank”

WITNESSETH: That-

WHEREAS, the BORROWER has a credit accommodation (Auto Loan) under Account
Number @AULLAASAL20R05089, amounting to PESOS : FIVE HUNDRED EIGHTY EIGHT
THOUSAND ONLY (Php 588,000.00) with the BANK which loan account has been restructured,
and the payments for which are covered by the Post-Dated Checks (PDCs) representing the
monthly amortizations of the said Auto Loan at Php8,606.21 (accrued interest) for 3 months
and Php15,804.05 for the remaining months.

WHEREAS, the Borrower is not able to submit the required number of PDCs due to non-
availability of newly requested checkbook. Thus, the BORROWER is requesting to submit instead
three (3) monthly PDCs dated November 06, 2020 to January 06, 2021 in the amount of PHP
8,606.21, forty-six (46) monthly PDCs dated February 06, 2021 to November 06, 2024 in the
amount of PHP 15,804.05 each and one (1) PDC dated December 06, 2024 in the amount of
PHP 110,628.35 and the remaining seven (7) checks in the amount of PHP 15,804.05 each
dated December 06, 2024 to June 06, 2025 to be submitted before November 06, 2020.

The Undertaking stated herein is fully enforceable against the BORROWER, his properties and
his successors-in-interest and assigns.

Failure on her part to fulfill the above-described undertaking, within the period aforestated shall
render the restructuring agreement ineffective, and the loan shall revert to its past due status.

IN WITNESS WHEREOF, the parties have hereunto caused this Deed to be executed on
the day and at the place hereinabove set forth.
