CASE SCENARIO

FULL TERM BABY


Mrs. Maria Delos Santos, 25 years old live in Sn. Vicente Tarlac City. She works
as an office clerk, just married to Mr. Peter Delos Santos, a city engineer. Lately
she is experiencing severe morning sickness and hyperemesis gravidarum. She
then seeks medical advice under Dr. Aubrey Pablo on November 10, 2021 and
found out that she is 6 weeks pregnant for her first baby.

During her pregnancy she always comply with her pre-natal checkup, she had
her Tetanus toxoid immunization, have vitamins like:
1. Folic acid 600 microgram daily
2. Iron 27 milligram daily
3. Calcium 100milligram daily
4. Vitamin D 4000 units daily
5. Docosahexaenoic acid (DHA) 1000 milligram
6. Iodine 150 microgram daily

Last August 18, at 8am she experienced contractions or tightening, a "show",


backache, an urge to go to the toilet and leaking of her bag of water. Mrs. Delos
Santos is aware that these are all signs of early labor, so she was rush to Central
Luzon Doctor’s Hospital under the care of Dr. Pablo and on that same day
around 9PM she delivered an alive baby Boy through Normal Vaginal delivery. At
1 minute of age, the infant is alert and active, and has a strong cry. He has a
heart rate of 85 and a respiratory rate of 50. The infant's arms and legs are
flexed, the color of his body is pink, but the color of the feet is blue. The nurse
continues a physical assessment of the infant looking for normal and abnormal
findings. With APGAR SCORE of 9. her baby's temp is not within normal range,
which is 36.39 C. The infant should remain in the radiant heat warmer until her
temperature has stabilized, At 6hours of Baby Boy Delos Santos life his
temperature is 37.2C. Erythromycin eye ointment and Vitamin K is given

August 20, at 10am Mrs. Maria Delos Santos was discharged and her baby boy
in stable condition, with follow up schedule. Health teachings were given by
Nurse like cord dressing, feeding, and diaper change.
