Dear Sir/Madam,

Good Day! I, Beaflor J. Caldozo, newly graduate resident physician and with a passport no.
P3007235C, would like to request a tourist visa for myself.

I have travelled to Japan last October 5, 2019 to October 10, 2019 and I absolutely loved my trip
to Japan, which was concentrated only at Osaka, as it was very memorable. I will be traveling to
Japan again starting July 7, 2023 until July 21, 2023 with my boyfriend and his family. The sole
purpose of this trip is for leisure and there are places in Kyoto, Narra and Tokyo that I haven’t
visited yet. It is supposed to be our celebration of his birthday and anniversary which are both
in July 19 and 21, 2023 respectively. The reason I am requesting for a multiple entry visa is
because me and my boyfriend are planning to get married this year 2025 and would like to do
our honeymoon in your beautiful country during autumn. We plan to experience the four
seasons in Japan if we could find time and cheap airfares again. Japan is a rich in culture
country, that I want to spend more time learning from.

Regarding my Income Tax Return, I am recently graduated from my training as Internal Medicine
Physician and recently started work as a contractual physician. I filled my registration this May
and would secure ITR in July 2023.

Rest assured that my finances are enough to cover my trip expenses. We already secured our
play to stay from Osaka, Kyoto and Tokyo thru Airbnb. I was able to set aside P108,477 in my
Bank of the Philippine Island for this trip. I believe that is enough for me to thoroughly enjoy
myself for sight seeing and shopping within the duration of our trip. In line with this, I included
my bank certificate and proof of payment for our accommodation.
