AFFIDAVIT OF ABSENCE OF PARENTS

I, AILEEN R. MENDEZ, of legal age, Filipino and a resident of Brgy.

Cadac-an, Gamay, N. Samar, after being sworn to in accordance with law,
hereby depose and say:

1. That I am the biological daughter of Francisco R.

Romances and Elisa C. Castillo, who are residents of
Brgy. Cadac-an, Gamay, N. Samar.

2. That my parents have already passed away. My father

died in 1986 while my mother died on August 9, 2020.

3. That during their lifetime, my parents forgot to timely

register the fact of my birth before the Office of the Local
Civil Registrar of Gamay, N. Samar due to ignorance of
the provisions of the law.

4. That I executed this affidavit to attest the truthfulness and

veracity of the foregoing facts and in order that the fact of
my birth be recorded and registered in the Local Civil
Registrar of Gamay, N, Samar and for whatever legal
purpose this may serve.

IN WITNESS WHEREOF, I hereunto set my hand this 23 rd day of

August 2021, in Gamay, Northern Samar.
