Comprehensive Foster Care Giver Waiver

Please Read Each Statement, Thank you for volunteering and reviewing this waiver. If you agree to the terms, place your initials next to each statement. Then, print your name and sign the Comprehensive Waiver in the space provided at the bottom. Mail to Kim Field at the address in your materials.
Waivers ______I have been provided with an electronic or printed copy of a Foster Guide (Also called Virtual Binder), and I have read and agree to the requirements specified in this foster guide. ______I have been provided with information about the IGRSTX Yahoo groups for foster volunteers, and agree to join this group in order to receive relevant information and updates. In addition I understand that there are files on the IGRSTX Yahoo site that contain relevant rescue documents (like adoption agreements, etc) that I may view or download as needed. ______I release IGCA from any liability to my home or pets while a foster dog is in my care. The foster dog will be treated as a pet at all times. I agree to prepare the dog for home life by house training, teaching basic manners and teaching the dog how to be home alone. I understand IGCA will supply heartworm medication monthly and veterinary care upon approval. I agree to provide the same high quality dog food as I feed my own dogs. ______I agree to purchase a regular buckle collar or day collar on which to place the tags, and will leave this collar on at all times. Rescue will provide me the no-slip collar to use when walking my foster IG. ______I understand that IGs are high flight risk dogs. When the foster dog leaves my house, both the regular buckle collar, with ID Tags, and martingale collar with a lead attached to the martingale collar, will be on the dog. I understand that whenever a door to the outside is open, the foster dog will be restrained, either in a crate, on a lead, or in my arms. I understand an IG can squirm through my legs and be gone. ______I agree to always closely supervise my foster dog in any area with access to a pool. I will never leave a foster dog unattended near an unfenced pool, even for a few moments. ______I understand that the prices charged by my preferred vet may exceed those allowed by South Texas IGCA guidelines. If, despite this, I strongly prefer my foster dog be cared for at that clinic rather than an approved rescue vetting clinic, I agree to be financially responsible for any charges incurred that are over and above the current rate allowed for that treatment. A list of these amounts will be provided to me upon my request and prior to treatment. _______From time to time, fosters request permission to have the Bordetella vaccine given to dogs in their care. Rescue will not reimburse for it [except under certain conditions at WAAC or ask your rep or vetting coordinator], but because the vet they choose requires it or they feel it will help protect their own dogs, some fosters are willing to pay for a Bordetella vaccine out of pocket. There are risks associated with any vaccine. However, you may do so if you agree to: 1. Keep the dog in the vets office for at least 15 minutes after the vaccination is given, in case there is a reaction 2. Be responsible for treatment should a reaction occur, and agree not to seek reimbursement from IGCA for any related vet bills. ____________________ Foster Care Giver Name __________________________ Foster Care Giver Signature _________________ Date
