JOINT AFFIDAVIT OF LOSS

We, EVELYN CENO SORIANO and JERRY AUSTRIA SORIANO, both of legal
age, married, Filipino and with residence address at Phase 2 Lot 3 Blk. 35D Welfare
Ville Compound, Brgy. Addition Hills, Mandaluyong City, after having been sworn to
in accordance with law, hereby depose and state, that:

1. We are the regular subscribers of both Globe Telecom SIM Cards inserted in
one Cellular Phone, which are registered to the different owners listed as;

a. Evelyn C. Soriano registered owner of Globe SIM with

No. 0915-799-0089
b. Jerry A. Soriano registered owner of Globe SIM with No. 0915-674-
2823

2. The cellular phone holder of a Vivo phone unit Y91, Jerry A. Soriano,
accidentally dropped the phone without knowing it was lost in his pocket
when he went to public market sometime on May 23, 2023, Tuesday.

3. Effort were exerted to recover the said Cellular Phone and SIM Cards, but in
spite of diligent search, it could not be found and the same now beyond
recovery;

4. As such, we are executing this Affidavit of Loss to attest to the truth of the
foregoing and to support the issuance of a new one in lieu of those which was
lost, and to use the old numbers to be registered on new SIM Cards.
