MANPOWER SUPPLY SERVICE AGREEMENT

This Supply Agreement (“The Agreement”) is made on ……………………………………

between

………………………………………… private limited incorporated in Malaysia with registration no. …………………..

having its principal address at …………………………………………. (hereinafter called “THE CONSULTANT”) of
the one part;

and

…………………………………………… specializes in cleaning services only) having its principal address at

………………………………………………………………………………. (hereinafter called “THE COMPANY”) of the other
part.

NOW IT IS HEREBY AGREED AS FOLLOWS:-

1. GENERAL PROVISIONS

1.1 THE CONSULTANT is the only organization authorized to fulfill quota/ approval of THE
COMPANY and shall supply the manpower from Bangladesh/ Nepal required by THE
COMPANY according to availability of manpower supply from source countries. If
Bangladesh if unable to supply, THE CONSULTANT has all rights to supply worker from
Nepal.
1.2 The services of THE CONSULTANT shall be to source, recruit and supply to THE COMPANY
including preparation of a complete travel document papers for approval by Bangladesh/
Nepal Labor and Employment Department, Department of Foreign Affairs, and other
government agencies in Bangladesh/ Nepal and Malaysia, concerned with overseas
recruitment of Bangladesh/ Nepal workers, to secure necessary security clearance, valid
passports and travel clearance and assist in the scheduling and ticketing as and when
required by THE COMPANY.
1.3 THE COMPANY shall authorize THE CONSULTANT as its agent/representative in
Bangladesh/ Nepal and Malaysia in all matters involving the recruitment and hiring of
Bangladesh/ Nepal workers for its company.

2. TRAVEL ARRANGEMENT

2.1 THE COMPANY shall be solely responsible for the cost of securing visas and/or work
permits for the accepted workers.
2.2 If the worker is from Nepal, payment for air ticket (Kathmandu to Kuala Lumpur) will be
borne by THE COMPANY.
2.3 If the worker is from Bangladesh, payment for air ticket (Bangladesh to Kuala Lumpur) will
be borne by THE CONSULTANT.
 2.4 Air ticket price is subject to the current market price.

3. TERMS AND CONDITIONS OF AGREEMENT

3.1 Payment for Fomema, insurance, levy and other related expenses for workers’ valid travel
documents and/ or working permit are solely THE COMPANY’s responsibility upon arrival
in Malaysia until the completion of service. Payment breakdown is as follows:

3.2 THE COMPANY is responsible to provide accommodation in acceptable distance from the
workplace.
3.3 If the accommodation is not in acceptable distance from the workplace, THE COMPANY
is responsible to provide transportation for workers.
3.4 It is sole responsibility of THE COMPANY to look after the welfare of the accepted workers
including case of death and/ or repatriation.
3.5 It is understood that any problem that may arise or incur by the worker(s) such as but not
limited to accommodation, delayed and/or non-payment of salary and overtime pay of
workers and other factors beyond the control of THE CONSULTANT shall be the sole
responsibility of THE COMPANY.
3.6 THE COMPANY is responsible to pay for return air ticket (Kuala Lumpur to Nepal/
Bangladesh) upon completion of service which at least three (3) years.

4. SERVICE FEE

4.1 Service fee of MYR 1,300.00 (Ringgit Malaysia One Thousand Three Hundred Only) per
person is to be borne by THE CONSULTANT.
4.2 Service fee in inclusive of application, submission and collection of approval by Ministry
of Human Resources of Malaysia.
