I Sri. TAPAS DAS, son of Sri. Madhu Sudan Das residing at 95 P.N. Mitra Field Road, P.S-
Behala, P.O. – Alipore, District – South 24 Parganas, Kolkata – 700041, by faith – Hindu, by
Occupation – self-employed, by Nationality – Indian, do hereby solemnly affirm as follows:-
1. That I am the registered owner of motor cycle bearing Registration No. WB 20Q
3300, Chassis No. MD624HE3362L70279 and Engine No. OE3L62070193.
2. That I do hereby authorise my brother Sri. Bishnu Pada Dey, son of Late Sri. Madan
Dey, residing at 60. B.L. Saha Road, P.O – New Alipore, P.S- Behala, District – South
24 Parganas, Kolkata – 700053, by faith – Hindu, by Occupation – business, by
Nationality – Indian, to ride the said Motor Cycle as an when required by holding
proper and valid driving licence and also by carrying with the copies of necessary
documents and papers relating to the vehicle with him.
3. That my brother Sri. Bishnu Pada Dey vide Driving Licence No. 2020180172179
issued by Indian Union Driving Licence West Bengal State.

4. That I also authorise by said brother Sri. Bishnu Pada Dey to ride the vehicle (Motor
Cycle) abiding by the law and regulations of the land and also by obeying traffic rules. I
am giving this authority without any consideration or future expectancy.
5. The photograph and signature of my brother Sri. Bishnu Pada Dey is duly attested by me
hereunder.
6. That this authority will remain valid for 11 months from the date of it issue 21.03.2023.
