VERIFIED STATEMENT

I, Oliver Earl R. Aliño, of legal age, Filipino, with office address at

Rural Health Unit, Tabogon, Cebu hereby affirm that I am answering the
questions asked of me by Atty. Rhett Vincent M. Minguez, fully
conscious that I am doing so under oath and that I may face criminal
liability for false testimony or perjury.

OFFER OF TESTIMONY

The testimony of the witness is offered to prove the following:

1. That the witness attended Reymundo Pole Brocal on September

5, 2021 as a medico legal case;

2. That the witness found that Reymundo Pole Brocal suffered a
fracture at right upper canine and a 3 cm laceration at right upper
lip;

3. That in his opinion as a Medico-Legal Officer, the injuries suffered

by Reymundo Pole Brocal will incapacitate him from his
customary labor or require medical attendance for a period of 10
days unless complications act in or manifestations due to internal
injuries which are not apparent at the time of the examination
appear; and

4. Such other facts and circumstances relevant to the case and to

identify a document.

Q2 — What is your work?

A2 — I am the Municipal Health Officer of the Municipality of

Tabogon, Cebu.

Q3 — Have you met Reymundo Pole Brocal?

A3 — Yes.

Q4 — When did you meet Reymundo Pole Brocal?

A4 — I met him last September 5, 2021 because I examined him

due to a medico-legal case.

Q5 — Were the findings of the medico-legal examination put into

writing?

A5 — Yes.

Q6 — I am showing you a report/letter addressed to The

City/Provincial Fiscal stating in part that, “I have the honor
to inform you that BROCAL, REYMUNDO P, 64 years old;
male a medico-legal case has been attended to in this
Health Center”, is this the document which contain the
results of the medico-legal examination you conducted on
Reymundo Pole Brocal?
