Dear sir or madam:

We are the engineers responsible for the design of the mechanical [and electrical] work for this proposed new
[addition to an existing] project. We are enclosing three site plans which give a detailed location of the proposed
structure and also our desired location for the point of entry into the building. At this time our preliminary estimate
for this project is a maximum instantaneous demand of __________ gpm.
Based on the above data, we request the following information:
A. What are the size and location(s) of any existing water mains adjacent to the project site that can be utilized for
both domestic and fire protection water service? If at present no such service exists, what is the expected date
of completion for such services? Would you please mark up one copy of the enclosed site plan with this
information?
B. What is the depth of bury of the water mains based on the datum taken from the enclosed site plans?
C. What is the static and residual pressure in these mains? If there is any cost involved for obtaining this
information, or if you do not normally conduct such tests, please advise us. If weather conditions do not permit
such tests at this time, please advise us when such tests would be conducted.
D. Please advise us about the requirements of, and any specific required locations for, both domestic and fire
protection meter assembly installations. Is a separate meter required for both domestic and fire water service?
Please include dimensions for all such meter assembly installations.
E. Are there any requirements or preferred piping materials and jointing methods for this service?
F. Please provide us with a breakdown of the work provided by you, the utility company, and all work required to
be performed by the plumbing contractor relating to the domestic and fire protection service from the main into
the building and the installation of the water meter assembly. Who is responsible for connecting to the public
water main? If it is our contractor, please provide us with any rules and regulations regarding the installation.
G. Please provide us with any rules and regulations regarding the requirements for and installation of backflow
preventing devices. If you are not responsible for such installations, please provide us with the name and
address of the agency or department having jurisdiction in this matter. If there are none, please so state.
H. Based on your experience, would you please advise us of the minimum depth of bury acceptable for water
mains in your jurisdiction.

B. Do your regulations permit disposing of storm water into the sanitary sewer system?
C. What are your regulations regarding specific limits of pollutants of concern for disposal of industrial or
chemical waste into the sanitary sewer system? If you are not responsible for such installations, please provide
us with the name and address of any agency or department having jurisdiction in this matter.
D. What are your regulations for the installation of grease traps in your jurisdiction? If you are not responsible for
such installations, please provide us with the name and address of any agency or department having
jurisdiction in this matter. If there are none, please so state.
E. Are there any storm water management programs in effect for your jurisdiction? If you are not responsible for
such installations, please provide us with the name and address of any agency or department having
jurisdiction in this matter. If there are none, please so state.
F. If there are no sewers available to the site, please provide us with the rules and regulations for private sewage
and storm water disposal. If you are not responsible for such installations, please provide us with the name and
address of any agency or department having jurisdiction in this matter. If there are none, please so state.
G. Please provide us with a breakdown of all work required to be performed by the plumbing contractor relating
to the installation of the sanitary and storm water house sewer from the building into the public sewer. Who is
responsible for connecting to the public sewer? If it is our contractor, please provide us with any rules and
regulations regarding the installation.
H. Are there any unique requirements for the use of any piping material and jointing methods required for
installation of the normal sanitary and storm water piping in your jurisdiction other than those listed in the
applicable code?

We are enclosing a copy of the site plan which gives the location of the proposed structure and our
desired location for the point of entry into the building and our preferred location of the gas meter
assembly. At this time we estimate that this project will have a maximum instantaneous gas
demand of _____________CFH (BTUH).
The total gas input in CFH of all connected devices is as follows:
A. Device. Range of pressure is ________ to ________ in. wc. [in. Hg] [psi].

anticipate any problem with this location?
We request the following information:
A. The size and location of any existing gas main adjacent to our property that we can utilize to
supply our project. If at present no such service exists, what is the scheduled completion date?
B. What is the BTU content, specific gravity, and pressure of the gas in the main from which you
intend to supply our project?
C. Will your regulations permit [or require] a meter to be located inside the building, in a pit
outside the building, or exposed, adjacent to the building? Could you also provide a
dimensioned drawing, both plan and section, of the proposed complete meter assembly?
D. Please provide us with a breakdown of the work provided by you and by the utility company
and the work to be performed by the plumbing contractor in conjunction with the service
extension from the main into the building and any ancillary work to be performed by the
owner, such as concrete pad, telephone connections, electrical connections, etc., and the exact
point of connection of the building piping with your service and meter asssembly. Who will
make the final connection?
