I, MOEMINAH A. NUL, of legal age, Filipino, widow, and a resident of Prk. Omar Shariff Brgy. RH-
3, Cotabato City, Philippines, hereby authorizes MAISARAH A. NUL, also of legal age, Filipino,
married, and a resident of Prk. Omar Shariff Brgy. RH-3, Cotabato City, Philippines, Philippines, to
conduct the following acts in my behalf:

1. Inquire and/or follow-up, for my Survivorship Pension Benefits with

the Employees Welfare and Benefits Division (EWBD) of Supreme
Court, at Padre Faura Street, Ermita Manila as the surviving spouse of
late ABDILAH K. NUL, former Shari’ah Ciruit Judge of Datu Piang,
Maguindanao and compulsorily retired on May 31, 2018;

2. Submit necessary documents needed in compliance to claim the said

Survivorship Benefits;

3. Sign, execute documents pertaining thereto;
