JOB ORDER

KNOWN TO ALL BY THESE PRESENTS:

THIS CONTRACT OF SERVICE is entered into this 1 st day of August, 2022 at Prk. 8, Bitaugan, San Miguel
Surigao del Sur by and between the Department of Education – Tubakon Elementary School of Living Tradition,
Tubakon, Bitaugan, San Miguel, Surigao del Sur represented herein by the Teacher In-Charge JOSEPH S. TERSE,
FILIPINO, of legal age single and is resident of Badong,Tago , Surigao del Sur and herein the JOB ORDER WORKER.

MAY JADE M. JUSTO

Filipino, of legal age and a resident of Prk. 8, Bitaugan, San Miguel Surigao del Sur therein after called as JOB
ORDER WORKER.

WITHNESSETH
The school engages the services of MAY JADE M. JUSTO as Job Order hereby offer under the terms and
conditions:

1. The JOW shall render the following Job Description for the period covered from August –December 2022 in a
quarterly basis in the amount Six thousand pesos (6,000.00) per quarter to wit;
a. Take charge of the Janitorial Services of the school premises in every day clean.
b. Keeping the School are a clean in everyday when it needs.
c. The JOW shall render all day with time of 7:30 am to 10:00 am and 3:00 pm to 5:00 pm.
2. That job order shall not be titled to any other benefits such as ACA, PERA, and all other benefits.
3. That this contract shall be for the period of one year and may be renewed upon agreement of the both
parties.
4. That the funds for the payment of Job Order Contract shall be against school MOOE.
5. That this contract shall take effect 1st day of August, 2022 and shall be terminated upon its expiration.

The school engages the services of ARA D. NGOHO as Job Order hereby offer under the terms and
conditions:

1. The JOW shall render the following Job Description for the period covered from January –December 2022 in
a quarterly basis in the amount Three thousand pesos (3,000.00) per quarter to wit;
a . Take charge of the Security Services of the school premises in every day clean.
b. Keeping the School area clean in everyday when it needs.
c. The JOW shall render all day with time of 7:30 am to 10:00 am and 3:00 pm to 5:00 pm.
