Evaluation Instrument

For this evaluation instrument, a Google Form would be used to allow all educators to complete
the form. They would all be encouraged to complete the form so that we can evaluate the
instructional technology coaching program. Guskey’s levels of evaluation of professional
development were used to create the Google Form. (Guskey, 2000) Each question was
assigned a specific level of evaluation. The levels are listed below.

Level 1: Participants’ Reactions Level 4: Participants’ Use of New


Level 2: Participants’ Learning Knowledge and Skills
Level 3: Organizational Support and Level 5: Student Learning Outcomes
Change (Guskey, 2016)

The Google Form would allow us to quickly and easily analyze the data that we receive. This
will allow us to evaluate the instructional technology coaching program in a timely manner. The
data will be used to evaluate the program based on all the levels. If one level shows to be
lacking then reevaluation of that portion will be needed.

2. Do you feel like it is a beneficial use of your time?
3. Does the instructional technology coaching program provide you with knowledge and
skills you can use in your classroom?
4. What skills will you use from the instructional technology coaching program?
5. Do you feel like you were provided with sufficient resources?
6. Were your questions/problems addressed in a timely manner and efficiently?
7. Have you implemented the new knowledge and skills in your classroom?
8. How have your students been affected?
9. Have you seen any results on student performance or achievement?
10. Do you feel like your students are more confident learners?
