AFFIDAVIT OF LOSS OF SIMCARD

I, Maiah Eliza R. Castañeda, of legal age, single, Filipino, and resident of
Dontogan, Green Valley, Baguio City, Philippines, after having been duly sworn to in
accordance with law, do hereby depose and state THAT:

1. I am a SMART Sim card user with Mobile Number 09083382548 since I

bought the same since then. The said simcard was inserted in my cellular
phone. Further, I had been using the said simcard for my G-Cash account
where transactions relative to the same were being made;

2. Sometime on the last week of August 2021, I went to the Comfort Room
purposely to relieve myself at the Clean Fuel establishment at Barangay Pico,
La Trinidad, Benguet along with me is my cellular phone where the SMART
sim pack which I was using for my G-Cash transactions was inserted, the said
comfort room is just beside my workplace. After that, I return and came back
to my work place believing that I have my phone with me and I did not forget
something or whatever. After my work is done, and were going home, I
remember my phone therefore I check inside my bag. It was then and there
that I noticed that the cellular phone aforementioned was no longer inside my
bag.

3. I immediately searched or locate the said cellular phone, wherein the simcard
which I was using in my G-Cash account transactions was inserted, within the
premises as well as going back to the Clean Fuel, and/or in other places
where I might have kept the same, but I was still I was unable to locate it.

4. Despite diligent efforts exerted, I was not able to locate the afore-mentioned
cellular phone, wherein the simcard which I was using in my G-Cash account
transactions was inserted, until now. Therefore, I considered it lost and
beyond recovery. The loss of the same is not deliberate but purely due to
mere inadvertence or excusable neglect;

5. I hereby bind myself to immediately notify and/or surrender the said items to
the proper authorities/company should later on the may be found,
nonetheless;

6. I am executing this affidavit to attest to the truthfulness of the foregoing facts

and for all legal intents and purposes it may serve well.
