I am allowing my daughter/son ANGELA KAYE GABASA to participate in the

( NSTP BASIC FIRSTAID TRAINING) on June 2, 2023 at 7:30-12 noon at Jaro Plaza Covered Gym,
as requirement in the NSTP/CWTS subject. I understand that the school has undertaken proactive
measures for safety and security of my child within their capacity. I will not hold the school responsible
for any incident that may occur due to the failure of my child in complying with the rules and regulations
stipulated by the school.
