Capiz specifically at Tapaz Central Elementary School, District of Tapaz West.

I am WAYNE P. SALUDES, 26 years of age, single, in good health and a resident of

Barangay Initan, Tapaz, Capiz.

As to my educational qualification, I graduated my Bachelor in Elementary Education at

Capiz State University - Tapaz Satellite College, School Year 2016-2017. I passed the
Licensure Examination for Teachers (LET) last September 2017 with a rating of 76.20%
at Iloilo Science and Technology University, Iloilo City Philippines.

This is to inform and respectfully request your good office that my rating in LET, Special
Training and Skills, Interview, Demonstration Teaching, and English Proficiency
Test obtained last School Year 2020-2021 be retained as my rating this School Year
2021-2022 except my Teaching Experience as my basis for the evaluation of applicant
in preparations for the New Registry of Qualified Applicants (RQA).
