SUMMER BOOK CHECK OUT

Prevent the “Summer Slide” by reading all summer long!

Students are invited to borrow books to read over
the summer.

To check out books, students must meet the following

requirements:
➔ Be a current MMS student
➔ Return or renew library books that were borrowed
during the school year
➔ Get parent/guardian permission by signing this form
➔ Return all books to the MMS or FA Library when
school starts in the Fall.

understand that the same MCSD replacement policy for lost/damaged books is in
