claim for liability against ACLC College of Tacloban including the Senior High School Department and its
Coordinator, the teachers handling this subject in particular and any of its representative, and release them from
any liability in connection to this activity. I expressly agree that in the event that my health, may make it
necessary to return home to take a rest at our own expense. I fully give my consent and will be responsible for
