AFFIDAVIT

I, MINDARITA A. ALCAZAREN, of legal age, Filipino Citizen, separated-in-fact

and a resident of Lower Sinawilan, Matanao, Davao del Sur respectively, after having
been duly sworn to in accordance with law, do hereby depose and say THAT:

I am the same and identical person who caused the execution of this instrument;

My husband SATURNINO D. ALCAZAREN and I got married sometime on

January 6, 1990. Out of our union came our three children namely: IRISH ALCAZAREN,
IAN LESTER ALCAZAREN and IVY ALCAZAREN;

My husband and I got separated sometime in the year 2003 or for almost 20
years now. Since our separation, my husband failed to give support and I was the one
providing for the needs of all our children;

We have no communication since our separation and I do not know his

whereabouts at present;

I am executing this Affidavit to attest to the truth and veracity of the foregoing
statements and for whatever legal purpose this may serve;

IN WITNESS WHEREOF, we have hereunto set our hands this 9 th day of June,
