I. Objectives
A. Content standard The learner demonstrates understanding of principles in the prevention and
control of communicable diseases for the attainment of individual wellness.

III. Procedures
A. Activity
1. Prayer The teacher will request everybody to stand up. Let someone lead the
prayer.
2.Greetings The teacher will greet the students. “Good morning class! How are you
today?”
3.Checking of The teacher will ask the leaders for each group if who are absent today.

4. Review The teacher will give the class a short recap about communicable disease,
Stages of Infection and type of communicable disease through oral
recitation.

3. Matt Kevin is a grade 8 student of Bung aw National High School. He
had a cough for almost 2 weeks now. One day while he spit some sputum
there is a streaked blood in the sputum. He also had a fatigue and he
cannot breathe properly. Matt Kevin suffers from what disease?
4. Aileen is a nurse in a prestigious hospital in the Philippines. During
pandemic, she is one of the frontliners in our country. One day Aileen got
a fever and lost her taste and smell. Based on the symptoms,what disease
does Aileen suffered?
5. Claire is absent in school for the past two days. Claire have a high fever
and her temperature is 40 degree celcius, she has headache, muscle bone
and joint pain, she suffered nausea, vomiting, pain behind her eyes,
swollen glands and she has rashes. Claire suffers from what disease?

5. Motivation The game called “UNLOCKED ME” . Students will be divided into five
(5) groups. Each group will have puzzled picture with corresponding
meaning of it. The group who will finish first will have 10 points.
B. Analysis *From the game “UNLOCKED ME” the teacher will introduce the topic
about agencies responsible for communicable disease prevention and
control.

*Divide the students into FIVE (5) groups. Each group will have an
activity sheet and express there understanding of the lesson by completing
the sentence starters below. Each group will be assigned one agency they
will answer.

D. Application Pass the Ball. Students will play pass the ball. They will pass the ball
to each other while singing one of the traditional folk songs in our
country the LERON LERON SINTA. The one who will have the ball
will answer the corresponding question.

IV. Assessment
V. Assignment Let’s widen your knowledge about the different agencies on
prevention and control of communicable diseases.
Directions: List and explain briefly at least 5 health programs that are
implemented in your community by the different agencies
on prevention and control of communicable diseases. Write
your answers in your activity notebook. The first item is
done for you.
