Application for Readmission

This is pertinent to students enrolled in a

graduate and undergraduate programs whose
status is absence without leave (AWOL).
However, AWOL status does not apply to the
Professional Teaching Certiacate program. PTC
students who failed to enroll for a speciac term
and would like to enroll for the next term just
need to send an email of intent to enroll to the
Program Chair furnishing the Registration
Section (registration@upou.edu.ph) a copy.
Sending the email of intent to enroll should be
done a month before the start of the
registration period.

Application Requirements:

1. Accomplished Application Form for

Readmission;
2. Application fee payment receipt
amounting to Php 225.00. See
procedures for payment of fees.

How to apply for for Readmission:

Step 1: Mail the accomplished form and payment

receipt to the O_ce of the University Registrar (OUR) at:

or email scanned copies to records@upou.edu.ph

before the set deadline for the term (refer to the
academic calendar for deadlines). Applications
received beyond the deadline will be processed
for the next term except those with the
endorsement from the O_ce of the Faculty of
Study (students with graduating status and those
who are scheduled to take the comprehensive
examination).

Step 2: Wait for the email notiacation from the Student

Records and Appraisal Section which will be sent to you
upon receipt of the evaluation result from the O_ce of
the Faculty of Study.
