CIRCULAR

Sub: correction of consumer Name/Address in database of LT consumers

With reference to the above, this is to inform you that various cases have been reported
regarding erroneous Consumer Name (Wrong Name, Wrong Spelling, etc.)/erroneous Address
printed in the Electricity Bill of APDCL in case of LT consumers and specifically consumers
under SAUBHAGYA. The consumers often fail to get their bills because of the erroneous name or
address and also failto get their names corrected due to lack of any standard procedure.
Further, due to such errors in the electricity bills the consumers often face problems while
submission of electricity bills as valid documents against various services.

Henceforth, the following procedure shall be followed for attending such cases:

a) The consumers can apply for "Name Correction" and "Address Correction" in the
standard form attached along with this letter.
b) In case of correction of Names, the consumer has to submit a self-attested copy of proof
of identity [any of the following, with authenticated photographs thereon) along with
the standard application:
i. Passport
ii. Voter ID Card
iii. PAN Card

iv. Government/Defence ID Card

v. ID Card of reputed employers
vi. Driving License
vii. Pension Payment Orders issued to the retired employees by Central/State
Government Departments, Public Sector Undertakings
viii. Photo ID Cards issued by Post Offices
ix. Photo identity Cards issued to bonafide students by a Llniversity, approved by the
Universigt Grants Commission (UGC) and/or an Institute approved by All India
Council for Technical Education (AICfE).

c) In case of correction of address

i. Passport
ii. Driving license
iii. Bank account statement
iv. Letterfrom reputed employer
v. Letterfrom any recognized public authority having proper and verifiable record of
i s su a n c e of su ch c ertifi cates

vi. Ration Card

vii. Voter ID Card (only if it contains the current address)
viii. Pension Payment Orders issued to retired employees by Government Departments,

both Public Sector Undertakings, ifthey contain current address.

ix. Copies of Registered Leave & License agreement/Sale Deed/Lease Agreement may

be accepted as proof of address.

d) The ESD/IRCA will verify the application form and the documents with the original
document of the consumer.
eJ After verification of the documents, the concerned SDE shall modify the name and
address of the consumer in ARMS database immediately.

0 The application and the self-attested document of identity/address proof submitted by

the consumer must be annexed with the original documents of the consumer submitted
during the time of service connection.
