INSTRUCTIONS
Pre-Activity:
“Are you ready?”
Submit your PAR-Q in Brightspace. In case there are “YES” answers, attach a medical
prescription signed by your physician.

“Warm-up”
Perform the warm-up exercises posted in Brightspace. Follow the mechanics properly.

Activity:
Fitness test
Perform a list of fitness tests posted in Brightspace. STRICTLY follow the mechanics
indicated in each test. Video-record your performance/take pictures as evidences and submit it
in our Brightspace.

Physical Activity Assessment


Using a Physical Activity Questionnaire posted in the Brightspace, assess your
engagement in physical activities.

Post-Activity:
Interpret the result of your fitness tests and physical activity assessment by comparing
them with the standards posted in the Brightspace. What should you do to improve your fitness
level?

Pre – assessment
What are the things you have to improve based on your interpretation of your current fitness
level? (1 to 5 sentences or bullet form)

Basing on the data gathered during the physical examination, I have found out that I should
improve on my flexibility (sit and reach) and endurance (forearm plank test). Both fitness test
garnered a poor rating indicating that I should be focusing on it. Now the remaining fitness tests
such as 3-minute step test, push ups and squat test should be maintained or much better be
improved.
