In May 2000, seven people lost their lives and over 2,300 became ill after drinking water
contaminated with Escherictiia coli Oi57:H7 and Campylobacter jejuni bacteria from
manure spread on a farm near Well 5 in Walkerton, Ontario. Schools, daycare centres
and businesses were shut down. The sports arena was converted Into a depot where
volunteers handed out bottled water and bleach, which It was necessary for them to
add to the water in Walkerton before using It to wash their hands. Helicopters flew
overhead transporting sick patients in air ambulances from local hospitals to a special-
ized hospital in London, Ontario.
Justice Dennis O'Connor concluded: "The most serious case of water contamina-
tion in Canadian history could have been prevented by proper chlorinatlon of drink-
ing water, according to a judicial inquiry report about Walkerton, Ontario's fatal £. coli
outbreak". His report also "points to the region's public utilities managers and Ontario
government cutbacks as contributors to the tragedy".

Introduction
It was 3 M a y 2000 a n d Stan K o e b e l , the m a n a g e r of t h e P u b l i c U t i l i t y C o m m i s s i o n
in W a l k e r t o n , w a s p r e p a r i n g t o leave for a c o n f e r e n c e in W i n d s o r , O n t a r i o .
L i t t l e d i d t h e r e s i d e n t s of t h e s m a l l f a r m i n g c o m m u n i t y k n o w that, o v e r t h e
next few d a y s , a series of faulty d e c i s i o n s that h a d b e e n m a d e b y Stan a n d h i s
b r o t h e r o v e r a l o n g p e r i o d of t i m e w o u l d c u l m i n a t e i n t h e d e a t h of s e v e n p e o p l e ,
i n c l u d i n g a t w o - y e a r - o l d c h i l d , a n d that n e a r l y half of W a l k e r t o n ' s p o p u l a t i o n of
4,800 w o u l d b e c o m e s i c k f r o m E. coli.

ment t o t h e P U C w e r e largely Ignored b y Stan K o e b e l , r e c o r d s of w a t e r t e s t i n g
h a d b e e n c o n t i n u a l l y falsified b y b o t h b r o t h e r s o v e r a l o n g p e r i o d of t i m e , a n d
i m p o r t a n t i n f o r m a t i o n was b e i n g w i t h h e l d .

W h a t factors c o n t r i b u t e d t o the final tragic o u t c o m e a n d w h y w a s n ' t s o m e t h i n g
d o n e s o o n e r ? T h e s i m p l e a n s w e r m i g h t b e t o l a y t h e b l a m e s o l e l y o n the K o e b e l
b r o t h e r s , b u t o t h e r s h a v e s u g g e s t e d that t h e c a u s e is m u c h m o r e c o m p l e x a n d
i n v o l v e s a s e q u e n c e a n d c h a n g e of events that c u l m i n a t e d i n t h e d i s a s t r o u s out-
come.

falsifying t h e r e c o r d s for y e a r s a n d n o t h i n g b a d h a d e v e r y h a p p e n e d . A l l he h a d
to d o w a s fill u p t h e s a m p l e b o t t l e w i t h t a p water, r e c o r d a n a c c e p t a b l e status
for t h e c h l o r i n a t i o n i n t h e l o g b o o k a n d n o b o d y w o u l d b e t h e w i s e r a n d h e c o u l d
go h o m e a n d e n j o y the rest of t h e day.
B y M o n d a y , Stan w a s b a c k f r o m t h e c o n f e r e n c e a n d h a d met b r i e f l y w i t h Frank,
w h o gave h i m a n u p d a t e of the events of t h e p r e v i o u s w e e k , i n c l u d i n g t h e infor-
m a t i o n that W e l l 5 h a d b e e n t h e p r i m a r y s o u r c e of w a t e r for W a l k e r t o n s i n c e the
p r e v i o u s T u e s d a y . After F r a n k left his office, Stan d e c i d e d that, e v e n t h o u g h Well
7 w a s s t i l l w i t h o u t a c h l o r i n a t o r , he w o u l d r e a c t i v a t e it, s w i t c h t h e s u p p l y back
to Well 7 a n d s h u t d o w n W e l l 5. H e a l s o h a d t o m a k e s u r e that w a t e r s a m p l e s
were c o l l e c t e d f r o m the w e l l s , s o t h e y c o u l d b e sent off for r o u t i n e tests the next
d a y t o A & L C a n a d a L a b o r a t o r i e s , t h e firm that h a d t a k e n o v e r W a l k e r t o n ' s water
testing w h e n G A P d r o p p e d the contract.
T h i n k i n g n o t h i n g m o r e of i t , Stan w a s n o t t o t a l l y s u r p r i s e d t o r e c e i v e a tel-
e p h o n e c a l l f r o m A & L L a b o r a t o r i e s o n W e d n e s d a y m o r n i n g i n f o r m i n g h i m that
t h e w a t e r s a m p l e that t h e y t e s t e d w a s c o n t a m i n a t e d . T h i s w a s n ' t t h e first time
t h e r e h a d b e e n p r o b l e m s w i t h the w a t e r s y s t e m i n W a l k e r t o n . A M i n i s t r y of E n v i -
r o n m e n t i n s p e c t o r h a d d i s c o v e r e d that W a l k e r t o n h a d t a i n t e d w a t e r between
1995 a n d 1998, a n d h a d m a d e r e c o m m e n d a t i o n s t o test m o r e f r e q u e n t l y a n d
to i n c r e a s e t h e c h l o r i n e levels i n t h e w a t e r s u p p l y . A l t h o u g h Stan h a d v o w e d
to c o m p l y w i t h t h e r e c o m m e n d a t i o n s , h e h a d n ' t d o n e a n y t h i n g . W h y b o t h e r t o
m a k e m o r e w o r k for himself? B e s i d e s , t h e w a t e r t a s t e d b e t t e r w i t h o u t c h l o r i n e .
N o t h i n g h a p p e n e d before, s o it w a s u n l i k e l y that t h i s s i t u a t i o n w a s a n y more
s e r i o u s . Stan k n e w that t h e r e p o r t i n g g u i d e l i n e s , w h i c h h a d b e e n c h a n g e d i n
1997, m e a n t that t h e results w o u l d n ' t go a n y f u r t h e r u n l e s s h e c h o s e t o reveal
t h e m . In t h e o l d d a y s , t h e M i n i s t r y of H e a l t h w o u l d h a v e t o h a v e b e e n i n f o r m e d
of a n y i r r e g u l a r i t i e s , b u t , w i t h n e w p r i v a c y r u l e s , t h e results of t h e w a t e r were
sent d i r e c t l y t o t h e c u s t o m e r , i n t h i s c a s e the P U C , a n d c o u l d o n l y b e p a s s e d o n
to t h e M e d i c a l Officer if t h e c u s t o m e r gave p e r m i s s i o n . S o Stan d e c i d e d t o d o
n o t h i n g , d e s p i t e a f o l l o w - u p fax o n T h u r s d a y b y A & L L a b o r a t o r i e s e m p h a s i z i n g
that W a l k e r t o n ' s entire s y s t e m w a s c o n t a m i n a t e d w i t h E. coli.
The whisteblower
O n S a t u r d a y e v e n i n g B o b M c K a y w a s feeling t e n s e . H e d e c i d e d he c o u l d s t a n d
it n o longer. H i s n e i g h b o u r s o n b o t h s i d e s h a d c o m e d o w n w i t h t h e m y s t e r i o u s
illness a n d w h o k n e w w h e n it w o u l d end? A s a long-time e m p l o y e e at t h e P U C ,
B o b k n e w that w h a t h e w a s a b o u t t o d o w o u l d m o s t l i k e l y j e o p a r d i z e his career
but w h a t c h o i c e d i d he have? H e h a d s e e n t h e fax f r o m A & L L a b o r a t o r i e s that
h a d a r r i v e d at t h e P U C t h e p r e v i o u s T h u r s d a y a n d n o w h e k n e w he h a d t o d o
something.

T h e o p e r a t o r i m m e d i a t e l y n o t i f i e d Dr. M c Q u i g g e , w h o w a s u p s e t w i t h t h e
P U C ' s s l o w r e s p o n s e b e c a u s e it h a d w a s t e d v a l u a b l e t i m e as m e d i c a l officials
c o n t i n u e d t o l o o k for f o o d s o u r c e s for t h e o u t b r e a k of E. coli. H e d e c i d e d t o c a l l
Stan K o e b e l , t h e manager i n c h a r g e of t h e w a t e r u t i l i t y at t h e P U C . B u t , first, h e
c o n t a c t e d his s e c r e t a r y t o i s s u e a n i m m e d i a t e " b o i l w a t e r a d v i s o r y . "

The epidemic
Stan slept w e l l S u n d a y night, b e l i e v i n g that he a n d F r a n k h a d the s i t u a t i o n u n d e r
c o n t r o l . B u t , w h e n h e a w o k e M o n d a y m o r n i n g a n d t u r n e d t h e r a d i o o n , t h e news

wasn't g o o d . T h e d e a t h of L e n o r e A l w a s b e i n g r e p o r t e d a n d t h e c a u s e w a s
b l a m e d o n c o n t a m i n a t e d water. T h e s e c o n d b i t of b a d n e w s f o r Stan w a s that
the M i n i s t r y of E n v i r o n m e n t w a s o p e n i n g a n i n v e s t i g a t i o n i n t o t h e town's w a t e r
p r o b l e m s . O n T u e s d a y , w h e n t h e M e d i c a l H e a l t h Office finally h a d c o n f i r m a t i o n
that t h e w a t e r s u p p l y w a s c o n t a m i n a t e d , Stan r e a l i z e d that h e h a d t o c o n f e s s t o
r e c e i v i n g t h e fax a n d a l s o that t h e c h l o r i n a t o r i n W e l l 7 h a d n o t b e e n w o r k i n g
a n d h a d not b e e n r e p l a c e d .
B y T h u r s d a y there h a d b e e n t w o m o r e d e a t h s . O n Friday, w h e n it w a s
a n n o u n c e d that a c r i m i n a l i n v e s t i g a t i o n w a s b e i n g l a u n c h e d b y t h e O n t a r i o P r o -
v i n c i a l P o l i c e , Stan a n d F r a n k b o t h k n e w it w a s o n l y a m a t t e r of t i m e before t h e i r
falsification of r e c o r d s w a s d i s c o v e r e d . T h e o n l y q u e s t i o n n o w w a s w h e n .
E v e n t h o u g h t h e s o u r c e of t h e o u t b r e a k w a s b e g i n n i n g t o b e u n d e r c o n t r o l ,
the E m e r g e n c y H e a l t h U n i t w a s t r y i n g t o s t e m t h e s p r e a d of t h e c o n t a m i n a t i o n
b y h a n d i n g out b o t t l e d w a t e r a n d b l e a c h f r o m t h e s p o r t s a r e n a , w h i c h h a d b e e n
t u r n e d into a t e m p o r a r y d e p o t . M o r e t h a n 40% of W a l k e r t o n ' s p o p u l a t i o n h a d
b e e n affected b y t h e b a c t e r i a a n d t h e v e r y i l l patients w e r e b e i n g airlifted t o
a s p e c i a l i z e d unit i n t h e bigger h o s p i t a l i n L o n d o n , O n t a r i o , b e c a u s e t h e l o c a l
hospitals were being stretched beyond their capacity.
T h e final c o u n t w o u l d b e that at least 2,300 p e o p l e b e c a m e i l l . T h e final d e a t h
t o l l i n c l u d e d s e v e n p e o p l e w h o d i e d d i r e c t l y f r o m E. coli, w h i l e c o m p l i c a t i o n s
from E. coli w e r e g i v e n as t h e c a u s e of d e a t h for a n o t h e r 14 e l d e r l y p e o p l e .

T h e I n q u i r y f o u n d t h a t i l l n e s s e s c o u l d h a v e b e e n p r e v e n t e d if S t a n K o e b e l h a d
m o n i t o r e d c h l o r i n e l e v e l s i n t h e d r i n k i n g w a t e r a n d i n s t a l l e d a c h l o r i n a t o r . B u t it
a l s o b l a m e d d e r e g u l a t i o n of w a t e r t e s t i n g a n d c u t s t o t h e E n v i r o n m e n t M i n i s t r y
b y t h e O n t a r i o g o v e r n m e n t as c o n t r i b u t i n g f a c t o r s t o t h e t r a g e d y . T h e bigger
q u e s t i o n is h o w w a s t h i s a b l e t o h a p p e n ?
Epilogue
O n M a r c h 25, 2003, S t a n a n d F r a n k K o e b e l w e r e " c h a r g e d w i t h p u b l i c n u i s a n c e ,
u t t e r i n g a n d f o r g e r y a n d b r e a c h of p u b l i c d u t y . " T h e b r o t h e r s p l e a d e d g u i l t y o n
N o v e m b e r 30, 2004, a n d w e r e s e n t e n c e d t o i m p r i s o n m e n t ; S t a n w a s s e n t e n c e d
t o o n e y e a r i n j a i l a n d F r a n k w a s s e n t e n c e d t o n i n e m o n t h s h o u s e a r r e s t . Stan
K o e b e l w a s r e l e a s e d after s e r v i n g o n l y f o u r m o n t h s .
1997: Walkerton—always responsible for its own water—has no set methods for certifying labo-
ratories, or a legal requirement to test water or report results.
June 18,1998: Walkerton Town Council complains to Ontario's conservative government about
cutbacks and the closing of labs, calling on the Ministry of Energy and the Environment to
ensure basic, healthy water standards for all Ontarians.
1999: Walkerton and two other local municipalities were amalgamated into Brockton.
April 22, 2000: Dave Biesentha follows proper practices and spreads 70 tonnes of cattle manure
over the field of his Stonegate farm, close to Walkerton's Well 5.
May 2, 2000: Activated Well 7 without installed operating chlorinator.
May 3, 2000: Stan Koebel instructs Frank Koebel to install the new chlorinator while he is
away.
May 5, 2000: Stan Koebel leaves for a water conference in Windsor, Ontario.
May 9-15: Well 5, a very shallow well, was the primary source of water for Walkerton.
Friday, May 12, 2000: Excessive rainfall swept southern Ontario, 70 mm (2.75 inches).
May 12, 2000 or shortly after: E. coli 0157:H7 and Campylobacter jejuni contaminants enter the
Walkerton system through Well 5.

Within a week: Walkerton residents and visitors begin to suffer symptoms of bloody diarrhea
and excruciating stomach cramps, head-fogging nausea, fever and chills, and sweats. Doc-
tors advise patients to drink lots of water
Saturday, May 13, 2000: Frank Koebel does not check chlorine residual at Well 5 during routine
check, falsifies the readings.
Sunday, May 14, 2000: Stan Koebel returns from conference to find that brother Frank has falsi-
fied records on status of Well 5.
Monday, May \5, 2000: Stan Koebel turns Well 7 on without chlorination at 1.15 pm.
Tuesday, May 16, 2000: Water samples received by A&L Canada Laboratories.
Wednesday, May 17, 2000; Patients start being treated for symptoms of cramps, fever, bloody
diarrhea, and vomiting. A&L Laboratories telephone Stan Koebel to inform him that the
water is contaminated.
Thursday, May 18, 2000: Stan Koebel receives a fax from A&L water testing lab stating that the
entire system was contaminated with E. coli. Koebel does not report the findings and tells
the mayor that the problem is not serious.
Friday, May 19, 2000: Dr McQuigge, Medical Officer of Health at the Bruce-Grey-Owen Sound
Public Health Unit, is notified by Dr Kristen Hallett, a pediatrician, of two cases of bloody
diarrhea referred from the Walkerton Hospital to Owen Sound Hospital.
May 19, 2000: Stan Koebel assures the Public Health Unit that the water is fine. The Public
Health Unit starts looking for a possible food source to explain the documented illnesses.
New chlorinator is installed on Well 7 and Stan begins flushing and superchlorinating the
system.
Saturday, May 20, 2000: The Health Unit's Environmental Emergency Center, Spills Action Cen-
tre (SAC), is informed of failed lab test results in the Walkerton water system taken from
a construction site by an anonymous call placed by Robert (Bob) McKay, an employee of
the PUC.
May 20, 2000: Forty more people report to hospital with bloody diarrhea and test positive for
E co/( 0157:H7.
May 20, 2000: PUC reassures the Public Healtfi Unit that Walkerton's water supply is safe.
Sunday, May 21, 2000: The Health Unit contacts Stan Koebel who assures them that the positive
samples were only from the construction site. Nonetheless, Dr McQuigge decides to issue a
"boil water advisory" to the people of Walkerton telling them not to drink the water
Monday, May 22, 2000: John Earle, a Senior Environmental Officer with the Ministry of the Envi-
ronment, Owen Sound District, is assigned to investigate the water problems in Walkerton.
May 22, 2000: Lenore Al, 66, is the first person to die from the contaminated water.
Tuesday, May 23, 2000: The Medical Health Office (MHO) laboratory confirms positive test
results for E. coli in the water. Stan Koebel finally tells MHO about the fax on May 18 and
admits that the machine that puts the chlorine in the town's drinking water supply did
not work for some time. Stan instructs Frank to clean up the operating log sheets for May
2000.
May 23, 2000: Edith Pearson, 83, and Mary Rose Raymond, 2, die.
Thursday, May 25, 2000: Drinking water system taken over by Ontario's Clean Water Agency.
Friday, May 26, 2000: Ontario Provincial Police launch a criminal investigation in Walkerton.
Wednesday, May 31, 2000: Conservative Premier, Mike Harris, reverses iiis earlier position and
calls a public inquiry. James Bolden, former mayor of Walkerton, visits local hospital with
symptoms. He reports that "while crowded, everything is running efficiently, including
flights to London Health Sciences Centre, which now holds 11 patients (ten of them chil-
dren)."
By June 1, 2000: Death toll has reached 11 with a further 784 treated for symptoms, including
90 who were hospitalized.
June 9, 2000: Ministry of Environment issues an Order requiring that Well 5 remain closed and
disconnected.
June 15, 2000: Death toll upgraded to 14. Seven more cases investigated by the coroner. There
are estimates that half of the population of Walkerton had got sick from the water contami-
nation.
July 25, 2000: Evelyn Hussey, 84, dies of kidney failure as a result of the E. coli poisoning.
December 20, 2004: Stan Koebel sentenced to one year in jail. Frank Koebel sentenced to nine
months of house arrest.
April 2005: Stan Koebel granted parole and released from prison after serving less than four
months of his sentence.
