Dear Sir,

Good day! I am Jhasmia Mabaya Revilleza, and I am interested to apply in the position of Teacher
III in Senior High School.

I earned my Bachelor of Science in Nursing at University of Southern Mindanao, Kabacan Cotabato

and passed the Nursing Licensure exam on the year 2010. I then took Bachelor of Science in
Secondary Education in Cor Jesu College, Digos City and took the Licensure Exam for Teachers in
2015 and passed. I worked as a nurse in Mabama Doctor’s Hospital, Bansalan Davao del Sur for 4
years. Now, I am currently employed at Matanao National High School-Senior High School teaching
Science subjects particularly Earth and Life Science and Physical Science for Grade 11 and Grade 12
students. I am also in in the track of Science Technology, Engineering and Mathematics (STEM). I
am in the teaching field from 2016 up to present.

I believe that my instructional abilities combined with my work ethics, skills and strong devotion to
educate children would greatly benefit the educational programs of our department.

I am hoping with great anticipation in which I can make a significant contribution to our division.
