Affidavit of the Complinant

I, WILLIANE GALIT BALDOZA, 38 years old, married, and a resident of Lot 11,
Block 56, Brgy Talipapa, Makati City, after having been duly sworn to in
accordance with law, hereby dispose and state that;

1. I am the wife of the victim, Jobellle Pontecilla Baldoza, 40 years old,

construction worker, and a resident of our place. A machine copy of the birth
certificate of Jobelle P. Baldoza in hereunto attached and made as an integral part
thereof as Annex"A".

2. That about 2:30 o'clock in the morning, Carlo Joaquin Katigbak called me for an
important things, that my husband was killed by the suspect, Joshua Lucapa
Pujante 35 years old, single, construction worker, and a resident of the said place.

3. That I was called to go to Makati City General hospital to look for my husband
in his critical conditions. And I don't know why he is in unconscious and I don't
have an idea if what happened.

4. That when I arrived at the Makati general hospital at about 3:00 o'clock in the
morning on October 29,2021, I was shocked that his body were on the morgue
and is ready to transfer to the autopsy for the cause of his death. The medico-
legal autopsy report, the NSO death certificate and a certification from the City
Civil Registrar Jobelle P. Baldoza in hereunto attached and made an integral part
of the complaint as Annex " B, C, and D." respectively.

5. That when I talk again to Mang Carlo I only know that my husband were dead
before the ambulance arrived and I don't know where the suspect hide and run.
The sworn statement of Carlo J. Katigbak is hereunto attached and made an
integral part of this complaint as Annex "E";
6. Mang Carlo immediately reported the killing incident on or about 7::00 O'clock
in the morning, October 30, 2021 at Makati City Police station. The original copy
of the excerpt of the police blotter duly issued by Patrolman Sandy Empalmado
Sacay of Makati City Police Desk is hereunto attached and made as an integral
part thereof as Annex "F".

7. I am therefore executing this affidavit freely and voluntarily in support of my

intent file a case of MURDER and/or the appropriate criminal case against Joshua
L. Pujante who is 35 years old and a resident of Lot 11, Block 56, Brgy Talipapa,
Makati City.

IN WITNESS WHEREOF, I have hereunto set my hand this 3rd day of November,
2021 at Makati City, Philippines.
