Method of Statement For Manholes

Scope of work,

The intention of this document is to describe the responsibilities of


involved, execution procedure for the concrete works at Sea port Yanbu AL-
Saniyah.

A. PLANING
1. Construction of concrete works should be carefully planned before work
starts so it can be carried out safely.
2. Planning involves identifying the hazards, assessing the risks and
determining appropriate control measures in consultation with all relevant
persons involved in the work.
3. Consultation should include discussions on the:
a. nature and condition of the ground and working environment.
b. nature of the work and other activities that may affect health and
safety.
c. site access.
d. type of equipment and tools used.
e. existing services and their location.

B. Man power.
1. Site Supervisor.
2. Safety Officer.
3. Lead Man.
4. Operators.
5. Labors.

C. Equipment and Vehicles.


1. Dyna Truck.
2. Boom Truck
3. Bacho Loader.
4. Bobcat.
5. Concrete Misture.
6. Vibrators.
D. Responsibilities.
Contractor will proceed and coordinate with concerned authorities to obtain
required work permit, supply the construction materials and control all site activities. The
Contractor will assign responsibilities to all personnel involved in the execution of the
project as follows;

1. Project Manager has overall responsibility for the civil works management
in accordance with the contract requirements.
2. Project Manager shall coordinate and control the execution of the civil
works, and ensure completion within scheduled time and the observance of
HSE and quality procedures and practices in accordance with the relevant
regulation of authorities.
3. Safety Officer has overall responsibility for the implementation of HSE plan
according to the Project HSE Plan.
4. Safety Officer shall investigate and report all major accident, recommend
protection measures, and ensure the availability of safety equipment and
first aid kits.
5. QA/QC has overall responsibility for the control and inspection of quality
according to approved ITP.
6. QA/QC shall review the Contract specifications and drawings, identify
project criteria and prepare required checklist for QA/QC inspection.
7. The Third Party Inspection company to which Contractor has
subcontracted the execution laboratory testing shall ensure that all certified
equipment meet the specified minimum requirements.
8. Site Engineer/Supervisor has overall responsibility for the implementation
of civil works in this specification.
9. Site Engineer/Supervisor shall arrange the manpower, equipment and
materials required for the implementation of the works, coordinate with
HSE officer and QA/QC Engineer involved in the tasks in accord to the
method statement and reporting.

E. Quality Control.

1. Testing shall be carried out in order to adequately demonstrate the


suitability and acceptability of the material.
2. The required inspection for each activity will be monitored by an authorized
engineer and certificated TPI (Third Party Inspection) company for Concrete
compression during construction.
3. All inspection report shall be submitted to Company according to above
mentioned.
4. All materials used in the works shall comply with the relevant company
specification.
5. Prior to using concrete, contractor shall submit the concrete trial mix design
for acceptance.
6. All the documents shall be maintained and filed. The records shall be
electronic and hard copies maintained at a readily accessible location for
review by the Department or client at any time. The documentation shall
include results for the delivery of materials, survey report, batched copy of
concrete, inspection reports, test reports etc.

F. Work Method for Concrete Work.


a. Reinforcement.

1. All metal for reinforcement should be free from loose mill scale,
loose rust, oil and grease, or harmful matter, immediately before
placing the concrete.
2. Bending machine or hand cutter with low steady pressure shall be
used.
3. All Bars will be 60 grade epoxy coated according to approved
MTC for all underground concreti ng and without epoxy
coati ng bars will be installed above the ground level.
4. Cut and bend all reinforcement as per drawings at work shop, and
will be fix at site, where necessary.
5. Bars will be transported to the Site for placing by trailer or truck.
6. All reinforcement should be placed and maintained in the position
shown on the drawings.
7. Bars shall be tied together using annealed iron wires PVC coated
at intersection points, to form rigid cages that will maintain their
position without distortion or twisting during the placing and
compaction of the concrete.
8. Prefabricated concrete spacers or plastic type spacer shall be inserted
between the bars and internal surfaces of the formwork, so that the
required concrete cover thickness is maintained during the placing
and compaction of the concrete. Where concrete blocks are used,
these should be made of mortar not leaner than 1 part of cement to
2 parts of sand.
b. Form Works,
1. The formwork should be so constructed as to remain sufficiently
rigid during the concrete placing and should be sufficiently tight to
prevent loss of cement slurry from the concrete.
2. The important qualities of good formwork relates to rigidity and
strength, economy of material, re-use, convenience of erection and
striking and good surface finish.
3. Execution of form work shall take into account climatic conditions,
the expected heat of hydration and finishing of concrete surfaces.
4. All edges of exposed concrete members shall have a chamfer of
approximately 25 mm, unless otherwise specified.
5. Form shall be sufficiently tight to prevent leakage of cement. All
surfaces of forms and embedded materials shall be cleaned of any
accumulated mortar from previous concreting and of all matter
foreign material before concrete is placed in them.
6. Before concrete placing commences, all wedges and other adjusting
devices shall be secured against movement during concrete placing.
7. Scaffolding will be installed where necessary.

c. Embedded items and water stop.

1. Anchor bolts, pipe sleeves, anchor rails, inserts, bolts, ladder etc.
shall be positioned accurately using a wooden or steel template and
be firmly attached to the form work before the concrete is poured.
2. Where bolts and anchor bolts are to be installed, additional rebar
shall be used to acquire an accurate position. Bolts and anchor bolts
shall be tied with an annealed iron wire on the additional rebar.
3. The elevation and plot plan position shall be checked before and
immediately after casting to ensure that embedment have not moved
during casting.
4. The location of water stop shall be as indicated on the drawings.
5. Each piece of pre-molded water stop shall be of the maximum
practicable length according to the site.
6. Adequate provisions shall be made for the support and protection of
water stop during the works.
7. The joint shall be properly cleaned, inserted and jointed in
accordance with manufacturer recommendations.

d. Concrete Placing.
1. The measuring, mixing, production and transporting of concrete shall
be through transit mixture and under no circumstances water shall be
added to the mixer in transit to the site.
2. Before each concrete pouring activity, the Contractor shall give
advance notice of 1 day so that an inspection may be made before
the concrete is placed.
3. When placing of concrete by pumping method, a backup plan shall be
in place to ensure the continuity of concrete placing in case the pump
should fail.
4. The total time required for mixing, transportation and pouring of
concrete should in general not exceed 45 minutes.
5. All concrete shall be consolidated by vibration, spading, rodding or
forking so that the concrete is thoroughly worked around the
reinforcement and around the embedded items.
6. Spare vibrators shall be kept on the job site during all concreting
operations.
7. Concrete pump shall be used for walls, columns and foundations
below the ground level.
8. For the pouring of concrete for walls and columns special care shall
be taken to ensure the concrete is fully compacted at the base.
9.  To maintain the drop height, a flexible hose shall be connected to the
outlet of the pump then it will insert inside the wall forms or by
specially designed drop holes around 200x200 mm shall be provided
in the shuttering at every 2meter C/C and at every 1.5 meter height of
shuttering to keep the minimum drop height.
10. Initial curing shall follow the finishing operation after 12hours, and
shall be kept continuously moist for 24 hours.
11. Provide approved bonding agents at construction joints if
construction joint occurred.
12. Repair work to be conduct when repair work occurred.
13. Take care of Housekeeping every day before leaving the site.

e. Concrete Placing in Hot season.


1. A first and very necessary step before pouring the concrete is to
thoroughly moisten the sub-grade, reinforcing steel and wooden
forms before placing the concrete.
2. Enough ice should be use during batching of concrete to reduce the
temperature of concrete.
3. Avoid delay in placing the concrete. Have sufficient labour and
equipment on hand to perform the placing quickly.
4. Don't order or try to place more ready-mixed concrete than you can
reasonably expect to finish and cover.
5. Care should be exercised with vibrators, to avoid over- vibration. Five
to fifteen seconds of vibration, depending on the depth of the
concrete, should give the desired compaction.
6. Try to shade the concrete from direct sunlight or cover the concrete
with polythene sheet.
7. In cases of extreme hot weather, it may be wise to start jobs in the
afternoon to take advantage of lower temperatures in the evening.
8. Sub-grade should be damp, but not muddy. Saturate beforehand
then sprinkle again just before concrete is placed.
9. Discharge concrete from waiting trucks as soon as possible. Heat
evolution from cement hydration and continuous agitation results in
temperature rises in the concrete which can cause a rapid loss in
workability.

f. Curing,
1. Portable water shall be use for curing.
2. Initial curing shall follow the finishing operation after 12hours, and
shall be kept continuously moist for 24 hours.
3. Final curing shall be continued for at least 7 days in the case of
structural concrete.
4. After initial setting of concrete for vertical structures & foundations
remove the formwork carefully to avoid the breakage of corners and
cover the concrete with burlaps and plastic sheet during curing
period to maintain surface wet.

g. Removal of Form work,


1. The form work shall be removed from column, wall, beam and other
parts not supporting the weight of the concrete after 24hours and for
supporting the weight of the concrete after 14 days or as per client
approval.
2. Formwork shall be removed carefully without shock or disturbance to
concrete.
3. After the removal of formwork immediately remove nails from the
wood to avoid hiding the nail in foot or other part of body.
4. Stacking of formwork shall be in good manner for reuse.

h. Concrete service protection.

1. Before applying the bitumen coating the concrete shall be cured, all
honeycombs properly patched with approved material and the surface
must be dry.
2. Before bituminous coating activity, the Contractor shall give advance
notice of inspection.
3. A protective bitumen coating shall be applied on the blinding and on
concrete surfaces below and 15cm above the ground level.
4. External concrete surfaces above ground shall be coated with approved
epoxy Paint System.
G. Safety Measures.
1. All the worker shall wear PPE at all times such as Safety helmets, safety
shoes, Gloves etc.
2. All the workers have to be briefed about the concreting work and safety
measures required.
3. The spoil shall be remove right before starting the Job and clean the site
properly.
4. Adequate enough safety barriers (Concrete Barrigate) to cover work area,
sign boards, advance warning signs would be used.
5. Sign boards to be kept at all visible places and to be protected.
6. Close supervision and recording by site engineer/ supervisor is required.
7. Place fire extinguisher / first aid box near the access.
8. All operatives shall be inducted before entering the site.
9. Tool box talk shall be conducted before the activity.
10. During formwork avoid scattering the nails and wood pieces and should
be removed immediately from the working area.
11. After the removal of formwork immediately remove nails from the wood
to avoid hiding the nail in foot or other part of body.
12. All the machinery has to be checked by the operators every morning
before the start of work and to fill the check lists with signature.
13. To avoid the collapse, the concrete Pump and concrete mixture should
park away from excavated area.
14. The speed limit of concrete mixture to be controlled. The drivers will be
responsible for all damages
15. All equipment should have his own fire extinguisher.
16. All equipment must have back horn and emergency light, also provide a
trained flag man to avoid the accident.
17. After the removal of formwork immediately remove nails from the wood
to avoid hiding the nail in foot or other part of body.
18. Remove the form work from the site to the yard.
19. Stacking of formwork shall be in good manner.
