These agreement was entered into Barangay Sta. Cruz, Guiguinto, Bulacan represented by
Hon. EMMANUEL S. JOSON, Punong Barangay, herein after referred to as the FIRST PARTY and
ARIES MATITU, of legal age, residing at Maunlad Homes, Sta. Cruz, Guiguinto, Bulacac referred
to as the SECOND PARTY.

WITNESSETH
That the FIRST PARTY needs the service of the SECOND PARTY as labor mechanic for the
Top Overhaul and repair of aircon of barangay vehicle Toyota Hi-Ace Van SGJ-103.

That the SECOND PARTY is the one who will undertake the services stated above;

That the FIRST PARTY will pay the amount of Seven thousand pesos only (P7,000.00) as
total payment for the services rendered of the SECOND PARTY.

That the FIRST PARTY and SECOND PARTY hereby agree and accept all terms and condition
of this agreement and bind themselves to comply herein:

In witness thereof, we have hereunder set our hand this 26 th of August, 2019 at Barangay
Sta. Cruz, Guiguinto, Bulacan.
