AFFIDAVIT OF LOSS

I, LEO A. CUYNO, of legal age, Filipino, and a resident of Casanta-Soong Rd.,

Mactan, Lapulapu City, Cebu, Philippines, after having been duly sworn to in
accordance with law, hereby depose and state that:

1. I am the attorney-in-fact of REINE P. YARAS, JUAN ICOY YARAS, REGIE

YARAS ALBANO, and RENALY ICOY YARAS as evidenced by the Special
Power of Attorney hereto attached as Annex A;

2. The business of Virginia Yaras under the name of “VIRGIE’S EATERY”

located at the Public Market, Poblacion, Asturias, Cebu, Philippines, was duly
registered with the Bureau of Internal Revenue (BIR) with Tax Identification
No. 402-608-271-000 and was therefore issued a BIR CERTIFICATE OF
REGISTRATION (BIT FORM 2303);

3. Unfortunately, the original copy of the Inventory of Unused Sales

Invoices/official receipts and the Unused Sales Invoices/official receipts and
all other unutilized accounting forms (e.g., vouchers, debit/credit memos,
delivery receipts, purchase orders, and others) including business notices and
permits as well as COR, were lost due to the destructive calamity caused by
the Typhoon Odette which happened last December 16, 2021, in the province
of Cebu;

4. I am executing this affidavit to support the request with Bureau of Internal

Revenue (BIR) for the closure of the said business, to attest to the truth of the
foregoing facts, and for all its legal intents, effects, consequences, and
purposes.
