Objectives The following were the specific objectives on the conduct of the activity:
a. To conduct K-12 Basic Education Program End of the School Year Rites for School
Year 2021-2022
b. To follow the Strict Protocols of the IATF in conducting the Program.
c. To witness the conduct of face-to-face Moving up and graduation Ceremonies for
School Year 2021-2022.
HIGHLIGHT OF ACCOMPLISHMENTS
Impact / Results The identified objectives and targets for the activity were met. Specifically, the team:
1.The program was started by singing of National Anthem, it was done through multimedia
presentation and followed by a prayer done also through multimedia presentation.
2..All teachers and parents were able to witness the conduct of face to face
Moving up and graduation Ceremonies.
3.The program was managed and successfully done.
4. In general, teachers and parents were proud for the job well done.
POST-ACTIVITY EVALUATION
Participants’ Everybody was happy especially the graduates, the proud parents, and the teachers as
Feedback well.
Methodology: Posting announcement through social media with regards to the schedule of
pictorials and the conduct of the said activity.
Management of the Activity:
The activity was managed by the teachers.
Logistics Arrangements:
All expenses incurred in the conduct of this activity can be charged against MOOE Fund and
subject for accounting and auditing.

DOCUMETATION
SCHOOL MEMORANDUM
1. Pursuant to DepEd Order on Calendar of Activities for SY 2020 – 2021 in which all parents and other
stakeholders are gathered for a general assembly. Bangon ES 2 nd Quarter General Assembly shall be
conducted on June 18, 2021 from 1: 00 pm – 5:00 pm in the afternoon.
2. The objectives of the activities are the following;
a. To hold scheduled PTA General Assembly to present school current situations, provide DepEd
updates and policies, report PTA fund of 2021, implemented/completed projects and their
corresponding amount utilized and other School funds received from other stakeholders.
b. To present again the “No Read, No Pass Policy and 3Bs (Bawat Bata Bumasa)” of the Division
of Samar.
c. To discuss other matters relative to the implementation of Modular Distance Learning in the 2 nd
Quarter.
d. To discuss about monthly pintakasi reiterated to encourage the participation of non-4Ps
members. To ensure the 100% participation of parents, a master list of parents was created for
monitoring purposes.
3. The Activity Completion Report (ACR) will be prepared by the teacher.
4. The participants in the said activity are School Internal and External Stakeholders.
5. Participants are advised to wear facemask and face shield, observe social distancing and other health
Protocols for safety measures.
6. There shall be no registration for all participants. The expenses for the said activity shall be charged against
PTA Fund and subject for accounting and auditing.

MINUTES OF GPTA GENERAL ASSEMBLY HELD ON JUNE 18, 2021 AT BANGON ELEMENTARY SCHOOL,
BANGON, PAGSANGHAN SAMAR
AGENDA:
a. To provide DepEd Updates and policies, report PTA fund of 2021, implemented/completed projects and
their corresponding amount utilized and other School funds received from other stakeholders.
b. To present again the “No Read, No Pass Policy and 3Bs (Bawat Bata Bumasa)” of the Division of Samar.
c. To discuss other matters relative to the implementation of Modular Distance Learning in the 2 nd Quarter.
d. To discuss about monthly pintakasi reiterated to encourage the participation of 4Ps and non-4Ps
members.
The meeting was started at 1:00 o’clock in the afternoon spearheaded by Mrs. May M. Salile (School Head)
The activity progressed accommodating the participants registration/attendance at exactly 1:00 o’clock in
the afternoon which was regulated by Ms. Marita M. Egana, GPTA Treasurer followed by a prayer headed by Mrs.
Analyn M. Montero, Grade 2 Adviser. The singing of National Anthem was conducted by Mrs. Salvacion V. Gallo,
Grade 1 Adviser followed by singing of DepEd Samar Hymn that was conducted Ms. Mary Nylve Jane I.
Vetorico,Grade 5 Adviser. Thought for the Day was given by Ms. Mary Clare B. Ignacio, Grade 4 Adviser and the
statement of purpose was given by Mrs. May M. Salile, School Head.
The meeting proceeded with the PTA conference which was directed by Mrs. May M. Salile. Some issues
and Concerns were raised by the parents and have been addressed by the class advisers. Before the meeting was
adjourned, closing remarks was conveyed by Mrs. Alma C. Macawili, GPTA President.
With no more apprehensions discussed, the activity ended soundly at 5:00 PM.
