AFFIDAVIT

I, PUJA AGARWALA, daughter of Late Nepal Agarwala, aged about – 23 yrs, by faith – Hindu,
by Occupation – Self-Service, presently residing at Palashipara, Nadia, pin- 741155 do hereby
solemn affirm and declare as follows:

1. That my deceased father NEPAL AGARWALA died on 13.08.2014.

 2. That, correct and actual name of my deceased father is NEPAL AGARWALA which is duly
recorded in my Voter Card being no. WPL1602689 but in my Aadhar Card being no. 7556
3441 9835 and P A N Card being no. BTFPA2583P the name of my father appears as NEPAL
AGARWALLA instead of NEPAL AGARWALA and in death certificate the name of my father
recorded as NEPAL AGARWAL instead of NEPAL AGARWALA and also in my UTI Mutual
Fund vide Folio No. 502187161586 the name of my father mistakenly recorded as KAILASH
CHAND AGARWAL instead of NEPAL AGARWALA.

3. That, all NEPAL AGARWALA, NEPAL AGARWAL, NEPAL AGARWALLA, KAILASH CHAND
AGARWAL are same and one person not different persons.

That the statements made above are true to the best of my knowledge, belief and faith.
