AFFIDAVIT OF DISCREPANCY
(ONE AND THE SAME PERSON)

I, HERMES E. FRIAS aka “HERMES FRIAS SR.”, married, of legal age,

Filipino, with residence and postal address at 1015 Villa San Jose Subdivision, Brgy.
Cutcut I, Capas, Tarlac, after having been duly sworn to in accordance with law, hereby
depose and state: That-

1. I was born to SPS. VICENTE B. FRIAS and LIBRADA ESTRELLA-FRIAS on

March 27, 1942 and was given the name “HERMES E. FRIAS” as evidenced by
my Certificate of Live Birth which is attached herein as ANNEX “A”;
2. I married MAYO PRINCIPE-FRIAS on July 24, 1975 and we were blessed with
THREE children;
3. When my wife gave birth to our Third Child, we decided to give him my name,
“HERMES P. FRIAS JUNIOR”;
4. After the birth of my aforementioned child, HERMES P. FRIAS JR., I started to
adopt the name “HERMES E. FRIAS SR.” in order to avoid confusion. I was
thus customarily known to all of my family, friends, and acquaintances as
“HERMES E. FRIAS SR.”;
5. My true and correct name is “HERMES E. FRIAS” as indicated in my Certificate
of Live Birth. However, I have been customarily using the name “HERMES E.
FRIAS SR.” to distinguish myself from my child Hermes P. Frias Jr., and avoid
confusion;
6. I thus assert that based on the foregoing facts and records in my possession, that
“HERMES E. FRIAS”, and “HERMES E. FRIAS SR.”, both refer to one and
the same person;
7. I am executing this Affidavit to attest to the truth of the foregoing, and for
whatever other legal intent and purposes it may serve.
