STUDENT STATUS SURVEY

1. How would you state your physical condition? Answer in paragraph form. Rate your physical well
being, 10 being very good and 1 being not good (nagkasakit, got covid positive, weight fluctuate, etc.)
As of now, I would say my physical state is around 9/10, much better compared to its state the past
weeks/months. I had Covid last January, dengue for a week last July then had to undergo a series of anti-
rabies shots from August to September because of an attack from a cat. I have been monitoring my
weight and diet well and continue to keep myself healthy.

2. How would you state your mental condition? Answer in paragraph form. Rate your mental health
from 10 being very healthy and stable to 1 being unstable and needing help.
My mental health would be 6.5/10. I feel like I could be doing better or worse. These past years has
heightened my anxiety about the things around me, especially my college life (or lack thereof huhu). As
of now I am still having problems with uncertainties in my life such as the health and financial stability of
my family, etc. However, I am currently practicing cognitive behavioral therapy (CBT) wherein I combat
negative thoughts/anxieties to help calm and ground myself and to solve/avoid having to think awful
scenarios.

3. How is your set-up at home or wherever you are currently staying? Do you have internet access for
remote learning? How is the connection?  What gadgets are at your disposal for remote learning?
I have access to a laptop and internet, however there are days when the Wi-Fi would be down or will
suddenly disconnect me from synchronous classes. I will make sure to notify if we are experiencing
problems with the connection and will try my best to reconnect.

4. Are art and/or office supplies/materials available in your house or near your area?
The nearest art/office supplies stores and shops near me are ones that require me to commute or take a
car ride to. I usually go for that option if I have a sudden or urgent need to acquire such materials. If the
materials are limited there or I have time to prepare them, I usually just order from shops online.

5. How is your financial situation? For your art materials? Load for data for internet connection?
I would say we’re in the lower to middle part of the middle class. My older brother and older sister are
the ones with stable jobs in the household I live in. My mother is a housewife, so she relies on my
siblings and pension from my deceased father in order to pay for bills and other expenses. I finance my
own materials through doing commission-based projects. I do this to lessen the financial burden of our
family and to save up for a dorm if face-to-face classes resume or become frequent.
