SUBJECT : Allegedly Fake Sick Note (LC012/23)

Background and objectives:

The Loss Control Investigation Department received an email on the 21 February 2023 from Vuyani Buwe, staff
number: 10000721, Senior Superintendent at Belville Tansfer Station. Mr Buwe requested us to investigate a sick
note that was submitted by staff member Thulani Ndabi, staff number: 10059533, Worker at Belville Transfer
Station.

Objective

The objective of the investigation was to verify the authenticity of the medical certificate.

Findings:

 The practice number that was in question on the certificate has been verified it is legitimate this is
according to The Practice Code Numering System (A Divison Of The Of The Healthcare Funders)
 The doctor has been registered as a medical doctor since 1996.
 The Address of the practice has been verified it is legitimate.

Recommendations:

No further Investigation needed case closed.
