This Letter reflects an appointment of employment with The RAO HOSPITAL. The details of this appointment
and the terms of your proposed employment are contained in this Appointment Letter.

1. COMMENCEMENT OF EMPLOYMENT
Your employment will be effective on a date mutually agreed upon following your acceptance of this
letter. As the date of this letter, the hospital is proposing a hire effective from the date
……………………………….. Your employment is contingent on the results of a background check which
may include a criminal records check and reference checks, and may include a review of education,
work and medical history. If the results of your background check reveal information that is
inconsistent with our standards, this appointment may be rescinded or your employment with the
hospital may be subject to immediate termination of this Appointment Letter.

3. COMPENSATION PACKAGE
The details of your annual compensation package including basic salary, taxable and non taxable
allowances, certain benefits, perquisites, reimbursements and other statutory payments (the
“Compensation Package includes ESIC and other necessary deductions”) are set forth in Annexure ‘A’
to this Appointment Letter.

4. TAXATION
The Compensation Package will be subject to all required deductions, including, without limitation,
deductions for tax normally to be withheld by an employer under Indian tax law. You all be
responsible for filing of your personal returns and compliance with other requirements under India
tax law.

5. HOURS OF WORK
You will be required to work for such hours as are necessary to suit patient’s requirements,
availability of hospital staff in a variety of locations and for proper discharge of your duties. You
are expected to work additional days and hours, possibly including public holidays, as might be
required for performing your duties competently and to meet the hospital’s requirement.
As an employee in the full time employment of the hospital, you will be obligated to devote your
entire attention and effort to the furtherance of the service of hospital and to continually develop
your professional skills in the interest of the hospital and yourself. You shall not, during
your employment with the hospital, directly or indirectly engage yourself or devote your time or
attention to any employment, business, or position of monetary interest, other than that of the
hospital.

6. PLACE OF WORK
Your assigned work location is the Jaipur. The hospital may, after giving you reasonable notice,
transfer or assign your services to any place of business of the hospital that may presently be
operating, or which may subsequently be acquired or established, in any part of India.
The hospital may also assign you to any work or assign your services to any associate company,
branch, office, subsidiary or other companies, concerns, hospitals, or firms with whom the hospital
may make any such arrangement or agreement.

7. LEAVE POLICY and MEDICAL BENEFIT


All regular and confirmed employees are entitled for 12 Casual leaves.
Leaves of one category cannot be adjusted any other category of leaves or against any leaves taken
beyond provided limit. Employees would have to necessarily take written permission from their
relevant superiors at least one week before availing any such leaves ( unless wherever practically not
possible) failing which, all such leaves would be considered leaves as without pay.

8. Medical Benefits
Confirmation of appointment depends on Pre employment health checkup. It will be done at
hospital and cost will be given by hospital. Moreover an annual health checkup will be done. Medical
Benefit facilities will be provided as per the rules of the BHRC as applicable from time to time.
Hepatitis B vaccination will be done for all staff who will be in direct contact with patient. If you are
already vaccinated, vaccination declination, proof of vaccination and submitted to Human resource
department of hospital.

9. RULES AND REGULATIONS


You shall in addition to the terms and conditions of employment specifically stated herein, also be
governed by the rules, regulations and such other practices, systems, procedures and policies
framed, amended, modified or omitted by the hospital from time to time. You will also be governed
by statutory laws enacted by Central or State Government or Local Authorities as may be applicable
to you from time to time.

10. CONFIDENTIALITY
You hereby acknowledge that any violation of this agreement during your employment will subject
you to dismissal based on gross misconduct as referred to under Section 12 below.
In addition, the terms of this Appointment Letter are strictly confidential and should be treated as
privileged information between you and the hospital.
11. TERMINATION
Subject to applicable law, this Appointment Letter may be terminated by either you or by the
hospital by providing thirty (30) days prior written notice to the other party or by payment of a
month’s salary in lieu of notice in case of termination of employment by the hospital. In the event of
Employee leaving the hospital, the employee shall give the notice to the employer for the period
calculated in accordance with term which they have completed with the hospital that is one month
notice for every year completed with the hospital, subject to maximum six months notice.

12. ACCEPTANCE OF OUR APPOINTMENT


Please confirm your acceptance of this appointment by signing and returning the duplicate copy of
this Appointment Letter for our attention within five (5) working days of the date of this
Appointment Letter.
