withdraw the benefits, services, facilities and privileges with respect to or in connection with the e-Money account

and/or CLPPRS, whether specifically relating to the Agents or generally to all or specific transacting clients.

a. In consideration of the right to offer the services through the e-Money, the Authorized Agent shall pay an
Account Opening fee in the amount of:

 Php 8,000 – Micro Pack


 Php 15,000 – Biz Pack

b. In case ____ received a cancellation or amendment instruction on the transaction made by the Authorized
Agent, a cancellation or amendment fee in the amount of Php100 shall be charged against the Authorized Agent, to
be deducted from the latter’s E-Money Account

1. COMMISSION FEES - ____ agrees to pay Authorized Agent a commission fee for every valid ______ transaction
successfully performed by the Authorized Agent. The amount of the commission fee is attached hereto as Annex “A”.

2. ANTI-MONEY LAUNDERING REQUIREMENTS.

a. Outsourcing of the gathering of “Know your Customer” (“KYC”) documents and information- ____ shall
outsource to Authorized Agent the gathering of minimum information and/or documents required to be obtained
under the Anti-Money Laundering rules and regulations of the BSP. However, ____shall have the ultimate
responsibility for knowing the customer and for keeping the identification documents. For this purpose, Authorized
Agent warrants that it has a reliable and acceptable customer information system and training program.

b. AMLA PROCEDURES. The Agent shall be responsible for the following Anti-Money Laundering Act
(AMLA) implementing guidelines:

CUSTOMER IDENTIFICATION. The Agent shall establish and record the identity of its ______ remittance
clients. This shall be done by asking the client to accomplish the Agent’s transaction form and to present a
valid ID.

SUSPICIOUS TRANSACTION REPORTING. The Agent


shall report via email or SMS to PJLI c/o Partner Care the transactions that are considered to be
suspicious/fraudulent.

RECORD KEEPING. All identification information and/or documents shall be turned over by the Authorized
Agent to P______ after one (1) year.____ shall review the documents and conduct the necessary risk
assessment of the customer. Authorized Agent shall have the responsibility of safekeeping the documents
gathered provided that the customer identification documents shall be made available to ____or to the
competent authorities within 24-48 hours from the date of the request. If _____ will request for information or
details during a client complaint investigation, Authorized Agent is required to acknowledge the request within
24 hours and provide the information needed within 48 hours. If _____I is being audited by regulatory
institutions, information must be provided within 24 hours.

c. THIRD PARTY RELIANCE. ______, in facilitating the remittance service, shall rely on the customer
identification process or gathering of minimum information and face-to-face contact undertaken by the Authorized
Agent and on the latter’s representation that it has already conducted face-to-face contact with the client. For this
purpose, Authorized Agent shall execute a written sworn certification, copy of which is attached hereto as Annex
stating that: a.) Authorized Agent has conducted the requisite customer identification requirements in accordance
with the Anti-Money Laundering rules and regulations of the BSP and its own Money Laundering and Terrorist
Financing Prevention Program (MLPP), including the face-to-face contact requirement to establish the existence of
the ultimate customer and has in its custody all the minimum information and/or documents required to be obtained
from the customer; and b.) P_____ shall have the ability to obtain identification documents from the Authorized
Agent upon request without delay.
