(CONCEPCION D. CULIS) because we are family friends and we
have known her for several years;
2. That we also personally know that from available records

(Marriage Certificate of son Agripino D. Culis), indicates that her
name is “CONCEPCION DIVINO”;
3. That we attest that “CONCEPCION DIVINO” as stated in SSS

Form E-1 with SS Number 04-0779817-7, “MARIA
CONCEPCION SERABIA DIVINO” stated in the Certificate of
Death of Agripino Divino Culis, “CONCHITA DIVINO CULIS” as
stated in the SSS Form E-1 with SS Number 03-7745184-5 and
“CONCEPCION DE VENA” as stated in the PSA Authenticated
Copy of Certificate of Live Birth of Agripino Culis, is one and the
same person;
4. That in all other documents she is using “CONCEPCION

DIVINO” (CONCEPCION D. CULIS);
5. That discrepancies in names/ records are due to clerical error;
6. That we are executing this Affidavit in order to attest the truth of

the facts aforementioned, and for any legal purpose this may
serve.
