At 1147 hours LT Received an appointment, subsequently we liaised with Member reps
to notify our attendance and arrangement of survey
At 1530 hours LT Surveyor departed Jakarta for Rembang city , Central Java

On 04 March 2023
At 0400 hours LT Arrive at Hotel in Rembang city
At 0830 hours LT Informed by member reps that W.P Damage Survey has been
arranged with claimant to be conducted at 1200 hours LT
At 1130 hours LT Surveyor proceed to site
At 1210 hours LT Arrived at site and met Mr. Widodo
At 1210 – 1500 hours LT Conducted W.P Damage Survey, interview with Fisherman and
Gathering information related to incident.
At 1510 hours LT Completed survey, attending parties left site
At 1600 hours LT Arrived at Hotel and overnight
On 05 March 2023
At 0630 hours LT Surveyor proceed to Breakwater and fishing boat that sustained
damaged at Batu Naga Beach
At 0700 hours LT Arrived at site
At 0715 – 1400 hours LT Conducted measured & inspected to alleged damaged of break water
and followed with details inspection to 10 units of fishing boat
At 1430 hours LT Completed survey, surveyor back to Jakarta

At 0830 hours LT Informed by Member reps that Member pair has sailed out from
Rembang water bound to Tuban , shipboard investigation is schedule
to be conducted on 05 March 2023 when member pair
arrived at Tuban Anchorage, East Java.

At 0330 hours LT Surveyor proceed to Soekarno Hatta International Airport, Jakarta
At 0500 hours LT Arrived at Soekarno Hatta International Airport, Jakarta
At 0600 hours LT Departed Soekarno Hatta International Airport, Jakarta for Juanda
International Airport, Surabaya
At 0735 hours LT Landed at Juanda International Airport, Surabaya
At 0750 hours LT Continue trip to Tuban by land transportation
At 1030 hours LT Surveyor arrived at Tuban and waiting for member reps
At 1245 hours LT Member reps arrived at Tuban and subsequently we proceed to
Tuban launch boat station
At 1300 hours LT Arrived at Tuban launch boat station than continue trip by boat to
location of member pair
At 1410 hours LT We boarded member pair and conducted key meeting with
concerned parties
At 1420 - 1930 hours LT Conducted shipboard investigation, interview crewmember as well
as gathered information related to incident
At 1940 hours LT Disembarked member pair than return back to Surabaya city
At 2230 hours LT Arrived at Surabaya and overnight in hotel
On 06 March 2023
At 0600 hours LT Checked out hotel and proceed to Juanda International Airport,
Surabaya
At 0730 hours LT Arrived at Juanda International Airport, Surabaya
At 0820 hours LT Departed Juanda International Airport, Surabaya for Soekarno Hatta
International Airport, Jakarta
At 0955 hours LT Landed at Soekarno Hatta International Airport, Jakarta
