PREPARATION AND CHECKING OF SCHOOL FORMS FOR SY 2022-2023

Date: June 15, 2023

1. To ensure and to guide all teachers/advisers involved in the preparation, checking,

and updating of forms through LIS, this school is enjoined to revisit the following
DepEd Orders.
 DepEd Order No. 11, s. 2018 - Guidelines on the Preparation and Checking of
School Forms
 DO No. 54, s. 2014 - Guidelines on the Request and Transfer of Learners
School Record
 DO. No. 58, s. 2017 -Adoption of New School Forms for Kindergarten, Senior
High School, Alternative Learning System, Health and Nutrition and
Standardization of Permanent Records

2. The Checking of Forms of the School Priority Grade Levels (Kinder, Grade 6) is from
June 21 – 23 while in Division Checking of School Forms is on June 26-28 and for
other Grade Level in School Checking of Forms is from July 13-14 while the Division
Checking is from July 17-21.

3. The correctness of the School Forms will be verified and certified by the Chairman
of the School Checking Committee (SCC) and validated by the Division Checking
Committee (DCC).

4. Refer to Enclosure No. 1 for the members of the DCC, Enclosure No. 2 for the Group
of Schools, Enclosure No. 3 for the Number of School Days and Enclosure No. 4 for
the Overall School Checking Committee and Grade Level Checking Committee of
School Forms.

5. Please be guided accordingly.
