SANGGUNIANG PANLUNGSOD RESOLUTION NO. _______-2023

RESOLUTION AUTHORIZING THE CITY MAYOR HONORABLE______________ TO

ENTER INTO A MEMORANDUM OF AGREEMENT WITH (Name of Hospital) AND
OTHER PRIVATE ACCREDITED FACILITIES AS THE REFERRAL FACILITY ON
OUTSOURCED SERVICES AND DRUGS/MEDICINES/SUPPLIES FOR THE
IMPLEMENTATION OF KONSULTA PACKAGE OF REPUBLIC ACT NO. 11223 IN THE
CITY OF ____________________, ILOCOS NORTE.

WHEREAS, Section 11, Article XII of the 1987 Philippine Constitution declares that the
state shall adopt an integrated and comprehensive approach to health development which shall
endeavor to make essential goods, health and other services available to all the people at
affordable cost, priority for the needs of the underprivileged, sick, elderly, disabled, women, and
children shall be recognized. Likewise, it shall be the policy of the State to provide free medical
care to paupers;

WHEREAS, pursuant to the Administrative Order No. 277 dated June 25, 1996 issued by
the Office of the President, the National Government and the Local Government Unit are
directed to ensure speedy implementation of the National Health Insurance Program (NHIP);

WHEREAS, R.A. No. 7160, otherwise known as the Local Government Code of 1991
grants Local Government Units (LGUs) genuine autonomy to enable them to attain their fullest
development as self-reliant communities and make them more effective partners of the national
goals;

WHEREAS, Section 16 of the Local Government Code provides that every local
government unit shall exercise the powers expressly granted, those necessary implied there from
as well as powers necessary, appropriate as incidental for its efficient and effective governance
and those which are essential to the promotion of the general welfare within their respective
territorial jurisdiction;

WHEREAS R.A. 11223 otherwise known as the NATIONAL HEALTH INSURANCE

ACT OF 2019 provides for the creation of the new KONSULTA (Konsultasyong Sulit at Tama)
Package which shall provide health primary care benefit coverage and ensure affordable,
acceptable, available and accessible health care services for all citizens of the Philippines;

WHEREAS Section 6 of the National Health Insurance Act provides for the coverage of
all citizens of the Philippines in the National Health Insurance Program. The local government
units shall ensure and support among other things the presentation and enrichment of culture,
promote health and safety, enhance the right of the people to a balance ecology, encourage and
support the development of appropriate and self-reliant scientific and technological capabilities,
improve public morals, enhance economic prosperity and social justice to promote full
employment among their residents, maintain peace and order and preserve the comfort and
convenience of their inhabitants;

WHEREAS, Section 17 of the Local Government Code states that Local Government
Code states that Government Units shall likewise exercise such other powers and discharge such
other functions and responsibilities as are necessary, appropriate or incidental to efficient and
effective provision of the basic services and facilities;

WHEREAS, Section 4 of the Revised Implementing Rules and Regulations of the

National Insurance Act of 1995 as amended by Republic Act 9241, provides for the mandatory
coverage of the employed in the government and private sectors. The enrollment of the indigent
households and individually paying group which comprise the majority of Filipinos not covered
by health insurance shall be ensured.

WHEREAS, it is a dire need to address the increasing demand for affordable health care
benefits for the residents of the City of __________________, Ilocos Norte and ensure that all
members and dependents in any sector must receive due benefits from the services of the Rural
Health Unit or any accredited Konsulta provider they have chosen;

WHEREAS, in order to realize our vision to provide all citizens with the mechanism to
gain financial access to health benefits, local government units shall find ways to ensure that
prescribed policies are in place;

WHEREAS, the City of _______________, Ilocos Norte thru the Honorable

________________is willing to adopt and implement the Konsulta Package contained in R.A.
11223 and defined in PhilHealth Circular No. 2020-0022 effective December 19, 2020;

WHEREAS, the City Health Unit of the City of _______________, Ilocos Norte is not
capable particularly the staffing compliment and equipment to be accredited as Konsulta
Provider to serve the needed quality health care services for the constituents within the
jurisdiction of the LGU;

WHEREAS. In order to address the temporary deficiency of the City Health Unit of the
City of _____________________, Ilocos Norte and eventually qualified as accredited Konsulta
Provider of NHIP, there shall be a Referral Facility to be contracted thru a Memorandum of
Agreement and provide the laboratory tests needed by patients opted to be served by the CHU;

WHEREAS. the Referral Facility shall provide the following services to the patients
referred by the Konsulta Package Provider as follows:

DIAGNOSTIC LABORATORY TEST Unit Price T.A.T

1. Chest X-Ray
2. CBC with platelet count
3. Urinalysis
4. Fecalysis
5. Sputum Microscopy
6. Fecal Occult Blood
7. Pap Smear
8. Lipid Profile (with total cholesterol, HDL and LDL
cholesterol, triglycerides)
9. FBS
10. OGTT
11. ECG
12. Creatinine
13. HbA1c

WHEREAS, the Referral Facility shall provide the following Drugs/Medicine/Supplies

to patients referred by the Konsulta Package Provider as follows:
1. Amoxicillin Anti-microbial
 14. Simvastatin Anti-dyslipidemia
15. Enalapril Anti-hypertensives
16. Metropolol
17. Amlodipine
18. Hydrochlorothiazide
19. Lozartan
20. Aspirin Anti-thrombotics
21. Chlorpheniramine Maleate Anti-histamine
