The respondent is the mother who is a housewife in the family, she stays at home and married to a 35 years old tricycle driver. They are originally from Leyte and moved at Zapote Las Pinas last year. The head of the family, the father works around town as a local tricycle driver and is the only one who provides for the familys needs. The whole family usually eats 3x a day and sleeps 6-8 hours everyday. They do not skip meals and eats fish and vegetables most of the time, seldom eats meat. B. The responder, G.R. is the mother, who is a housewife, stays at home and do household chores and in charge in taking care of their children, their source of income is the father, G.R. a local tricycle driver in town, who has an income of 600800 pesos each day. They manage to budget their meals and other expenses like electricity and etc Home and Environment The respondents house is made of hollow blocks and cement, the house is currently on progress because it is unfinished, it is a two story house, and the husband is the one doing the construction at home, it is full of wood and raw materials which can cause injuries, the inside of the house looks the same as the outside, unfinished walls, unpainted ceelings with hanging light bulbs which is above the familys bed, a carpet in the middle of the living room with two sofas on the side and a 36 sony bravia lcd tv. The house is somehow unsanitary due to its present condition, it is full of smoke and dust and no proper ventilation. It has only one window and one passageway. The kitchen is located at the corner of the house and no existing exhaust fan which causes foul smelling odor when cooking, and the kitchen is located parallel to the toilet which is unsanitary. The kitchen has no

refrigerator to store foods. Their location is just a few steps from the health center which is accessible for them when they need medical help. C. Health Status of Each Family Member The health status of each family member is somehow good, because the mother and father is able to provide each member of the family three meals each day and the mother is able to avoid vices in order for her to focus on taking care of her children and herself. D. Values, Habits, Practices on Health Promotion, Maintenance and Disease Prevention The respondents priority is her children, she stays at home as a housewife and does some household chores everyday like cooking, doing laundry, washing the dishes and most of all, taking care of her children. Her method of disease prevention is primarily on their nutritional needs, by eating healthy three times each day and not skipping a meal.

II. Gordons Eleven Functional Area A. Health Perception Health Management The respondents family seldom experiences any kinds of diseases, except for mild cough and colds, she seldom visits the health center because she is able to manage her familys illnesses independently like, increasing fluid intake and taking paracetamol and other means of health management. B. Nutritional Metabolic Pattern The respondents family eats three times a day and usually serves fish and vegetables, the seldom eat meat, their fluid intake is primarily water alone, they seldom drink carbonated or alcoholic beverages, except if there are any occasions. C. Elimination Pattern The respondents doesnt have any problems when it comes to elimination, she doesnt take any laxatives or any stool softeners, she defecates everyday in the morning and sometimes, twice a day including at night, she urinates depending on her intake of water. D. Activity Exercise Pattern The respondent walks around the house and does some household chores like, cooking, doing laundry and any other activities and these activities serves as her exercise, she goes out of the house when she needs to visit the market to do some groceries. During her free time, she watches television or sleeps for a few hours but most of the time, she spends her time with her children E. Sleep Rest Pattern The respondent and her family usually sleeps at 9 in the evening and wakes up at 4am to prepare her daughter in school. They sleep 7-8 hours everyday. F. Cognitive Perception Pattern

The respondent doesnt use any eyeglasses or hearing aids, she didnt finish her highschool and she is satisfied with her educational attainment and doesnt show any interest in pursuing her studies. G. Self Perception Self Concept Pattern The respondent is somehow satisfied with her life with her family, she is happy living with her husband and her children. H. Roles Relationship Pattern The respondent makes her decisions independently and sometimes, with the help of her husband and neighbors. She takes opinions and advices with no problems and respects each and every opinion. I. Sexuality Reproductive Pattern The respondent doesnt have any interest on being pregnant again yet, she just had a baby last year and it is too much for their income J. Coping Stress Pattern The respondent usually sleeps or watches television in order for her to divert her attention to their income problems, she takes advice from her neighbors. K. Values Belief Pattern The respondent and her family are all Roman Catholic, they attend mass every Sunday and seldom practices religious activities like praying the rosary.

A. Skin The respondents skin is fair, uniform in color, absence of any lacerations or lesions, respondents skin has good skin turgor when palpated. B. Head and Skull The respondents head is proportional and lined at the center of her body, absence of irregular masses and nodules. C. Nails The respondent has short pink nails and normal capillary refill. D. Hair The respondents hair is black, long, evenly distributed and absence of any foreign living organisms. E. Eyes The respondent has two eyes, symmetrical in size, and parallel to the ears, her pupils are equally rounded and react to light accommodation. F. Nose

The respondents nose is in the middle of her face, there are no irregular masses and deformities present and symmetrical in color to her face. G. Ears The respondent has two ears parallel in line with her eyes, with the same skin color of her face, her auricles are firm and the whole ear returns to its usual orientation when moved and palpated. H. Mouth and Oropharynx The respondents teeth is yellowish in color, the oral mucosa is pinkish and without any dentures, uvula is patent and presence of gag reflex, tongue can move freely and absence of lesions or any other mouth disease. I. Neck the respondents neck is lined at the center of her body, it is proportional to the side of her head and there are no lesions or irregular masses when palpated, the head can move accordingly and freely without effort or pain. J. Thorax and Lungs The respondent doesnt have any difficulty in breathing, breathing remains spontaneous and non labored. K. Breasts Not inspected. L. Central Heart Vessels Normal. M. Peripheral Vascular System The respondent has normal pulsations on her peripheral lines, extremeties is symmetrical in color with her whole body and no discolorations, maintains normal skin turgor and absence of edema. N. Abdomen Not inspected. O. Genitals Not inspected. P. Rectum and Anus Not inspected.
